SHOULDER IMPINGEMENT: REHABILITATION TREATMENTS AND UTILITY OF SPORTS PREVENTION

Abstract:

One of the most common pathologies of the shoulder, the most mobile joint of the locomotor system, is the so-called "subacromial impingement syndrome" or impingement syndrome, a progressive orthopedic condition, which occurs as a result of alterations in biomechanics and / or structural anomalies . The sportsmen most affected by this syndrome are athletes of sports that require the performance of repetitive gestures of abduction and external rotation such as in tennis, volleyball, handball, javelin throwing, but in reality it is a pathology also found in individuals who do not practice sport at competitive level. At the pathophysiological level, it can have different functional, degenerative and mechanical causes, including rotator cuff syndrome, tendinitis and shoulder bursitis. The treatment may be surgical and conservative, with the aim of restoring the joint movement; non-surgical treatment is used to reduce pain and subacromial inflammation, to heal the compromised rotator cuff and to restore a satisfactory shoulder function and should be considered as the first-line treatment. The aim of this thesis is a review of the possible rehabilitation protocols, for the treatment of shoulder impigement syndrome, with particular attention to both the possible personalized pathways and the effectiveness of physiotherapy and manual therapy, and to the underlying pathology, to the degree impairment, to the total range of motion, to the identification of shoulder pain. We will proceed with a quantitative and qualitative analysis of the literature studies, selected in order to have an overview on possible treatments such as manual therapy, physical therapy, strengthening exercises and targeted training protocols.

Abstract:

Una delle patologie piů comuni della spalla, l?articolazione piů mobile dell?apparato locomotore, č la cosiddetta ?sindrome da conflitto sottoacromiale? o impingement syndrome, una condizione ortopedica progressiva, che si verifica a seguito di alterazioni della biomeccanica e / o anomalie strutturali. Gli sportivi maggiormente compliti da tale sindrome sono gli atleti di sport che richiedono l?esecuzione di gesti ripetitivi di abduzione e rotazione esterna come nel tennis, nella pallavolo, nella pallamano, nel lancio del giavellotto, ma in realtŕ č una patologia riscontrata anche in individui che non praticano sport a livello agonistico. A livello fisiopatologico, puň avere varie cause funzionali, degenerative e meccaniche, includendo la sindrome della cuffia dei rotatori, tendiniti e borsiti della spalla. Il trattamento puň essere chirurgico e conservativo, con l?obiettivo di ripristinare il movimento dell?articolazione; il trattamento non chirurgico viene utilizzato per ridurre il dolore e l?infiammazione subacromiale, per guarire la cuffia dei rotatori compromessa e per ripristinare una funzione soddisfacente della spalla e dovrebbe essere preso in considerazione come trattamento di prima linea. Scopo di questo elaborato di tesi č una revisione dei possibili protocolli riabilitativi, per il trattamento della sindrome da impigment della spalla, con particolare attenzione sia ai possibili percorsi personalizzati e all?efficacia della fisioterapia e della terapia manuale, sia alla patologia sottostante, al grado di compromissione, all?arco totale del movimento, all?identificazione del dolore alla spalla. Si procederŕ con un?analisi quantitativa e qualitativa degli studi in letteratura, selezionati in modo da avere una panoramica sui possibili trattamenti come la terapia manuale, fisica, esercizi di rafforzamento e protocolli di allenamento mirati.
