The Bowel Paralysis Following Aboral Large Bowel Operations and Influence of Early Enteral Nutrition

Abstract:

Souhrn: Cílem studie bylo zhodnotit vliv časné enterální výživy na nitrobřišní tlak, obnovu peristaltiky, odchod prvních větrů a rychlejší odchod stolice. Pro výzkum jsem vybral resekci rekta, protože doba trvání pooperační paralýzy je delší než při ostatních nitrobřišních operacích a průměr konstruované anstomózy je exaktně určen průměrem použitého stapleru. Metodika: Pacienti byli randomizováni na sledovanou a kontrolní skupinu. Sledovanou skupinu s 30 pacienty tvořili operovaní, kteří dostávali oligomerní enterální výživu ihned po operaci. Výživa byla podávána trojcestnou sondou. Kontrolní skupinu 31 pacientů tvořili operovaní, kteří byli po operaci živeni parenterálně a narůstajícím časem od operace byli postupně živeni perorálně. Pooperační paralýza byla hodnocena pomocí křivky nitrobřišního tlaku prvních 48 hodin pooperačně. Nitrobřišní tlak byl měřen nepřímou metodou tzn. měřením tlaku v močovém měchýři. Výsledky: Křivky nitrobřišního tlaku, mediány nástupu peristaltiky, prvních větrů a stolice byly srovnány statisticky. Závěr: Prokázal jsem, že časná enterální výživa podávaná trojcestnou sondou po resekcích rekta snižuje nitrobřišní tlak rychlejším obnovením peristaltiky, dřívějším odchodem větrů a stolice.

Summary:Introduction: The aim of my reseach was to evaluate the influence of early enteral nutrition on the intraabdominal pressure, regeneration of peristalsis, first flatus and faster stool. For our research I have chosen rectal resection because bowel paralysis takes more time than other abdominal operations and the diameter of anastomosis with the used stapler is exactly determinated. Methods: The patients were randomised into observed and control group. The observed group consisted of 30 patients already operated on who received oligomer enteral nutrition immediately after the operation. Early enteral nutrition was given by nasojejunal tri-luminal tube. The control group consisted of 31 patients operated on whom we gave nutrition parenterally in the early post-operational period and during the progressing time period following the operation, nutrition will be given orally. The bowel paralysis was evaluated by intraabdominal pressure curve for first 48 hours after operation. Intraabdominal pressure was measured by an indirect method i.e. measuring pressure in the urinal bladder. Results: The curves of intraabdominal pressure and median of peristalsis, first flatus and stool was compared statistically. Conclusion: I proved that early enteral nutritinon given by triluminal tube decreased the intraabdominal pressure by quicker beginnig of peristalsis, first flatus and stool.
