Staphylococcus aureus from patients with cystic fibrosis - factors of pathogenicity

Abstract:

Diplomová práce je zaměřena na průkaz produkce některých faktorů patogenity kmenů Staphylococcus aureus izolovaných z dýchacích cest pacientů s cystickou fibrózou a z dýchacích cest kontrolní skupiny pacientů. U obou skupin byla také vyhodnocena citlivost k antibiotikům. Faktory patogenity byly vyšetřovány u 59 kmenů od pacientů s cystickou fibrózou a 60 kmenů od kontrolní skupiny. Citlivost k antibiotikům byla celkem vyšetřena u 59 kmenů od pacientů s cystickou fibrózou a u 55 kmenů od kontrolní skupiny. Vyšetřované faktory patogenity kmenů Staphylococcus aureus byly - produkce hemolyzinů, hyaluronidasy, volné koagulasy a biofilmu. Hemolyziny byly diagnostikovány na komerčně dostupných krevních agarech s ovčí krví a na v laboratoři připravených krevních agarech s propranými beraními a lidskými erytrocyty. Alfa-hemolyzin, beta-hemolyzin a delta hemolyzin byly prokázány u 78,0 %; 1,7 % a 74,6 % kmenů izolovaných od pacientů s cystickou fibrózou. Od kmenů kontrolní skupiny pacientů byly alfa-hemolyzin, beta-hemolyzin a delta hemolyzin prokázány v 78,3 %; 3,3 % a 80,0 %. Vyhodnocení neukázalo na zásadní rozdíl produkce hemolyzinů mezi kmeny obou skupin. Produkce volné koagulasy byla diagnostikována pomocí komerční diagnostické soupravy. Volná koagulasa kmenů od pacientů s cystickou fibrózou měla pozitivní reakci nejčastěji 2., 3., a 4. hodinu (66,1 %; 10,2 % a 10,2 %) a od kontrolní skupiny kmenů nejčastěji 2. a 3. hodinu (55,0 % a 31,7 %). Enzym hyaluronidasa byl prokázán dekapsulačním testem u 96,6 % kmenů od pacientů s cystickou fibrózou a u 100,0 % kmenů od kontrolní skupiny. Produkce biofilmu prokázaná u obou skupin kmenů Staphylococcus aureus byla přibliţně stejná, co se týče počtu biofilm pozitivních kmenů. Kmeny od pacientů s cystickou fibrózou byly biofilm pozitivní v 77,2 % a kmeny od kontrolní skupiny v 74,6 %. Kmeny od pacientů s cystickou fibrózou však byly v 31,6 % silně biofilm pozitivní, kdeţto kmeny od kontrolní skupiny byly silně biofilm pozitivní pouze v 15,3 %. Data citlivostí k antibiotikům byla získána z laboratorního informačního systému. Kmeny Staphylococcus aureus od pacientů s cystickou fibrózou (n = 41) v porovnání s kmeny od kontrolní skupiny (n = 55) vykazovali obecně vyšší rezistenci k vybraným antibiotikům, zejména k erytromycinu (48,8 %) a klindamycinu (41,5 %). Kmeny od kontrolní skupiny vykazovaly výrazně niţší rezistenci k erytromycinu a klindamycinu (23,6 % a 20,0 %). Výjimkou byla vyšší rezistence k ciprofloxacinu (30,9 %) u kmenů od kontrolní skupiny v porovnání s kmeny od pacientů s cystickou fibrózou (7,3 %). Z celkového počtu 59 kmenů Staphylococcus aureus od pacientů s cystickou fibrózou byly 3,4 % kmenů oxacilin rezistentních (MRSA), jednalo se pouze o dva kmeny. Z celkového počtu 55 kmenů Staphylococus aureus od kontrolní skupiny bylo MRSA kmenů 12,7 %.

Abstract:

The thesis is focused on demonstration of some pathogenicity factors of Staphylococcus aureus strains isolated from airways of patients with cystic fibrosis and from airways of control group patients. There was also evaluated antibiotic sensitivity of both Staphylococcus aureus strains groups. Pathogenicity factors were examined in 59 strains from patients with cystic fibrosis and in 60 strains from control group. Antibiotic sensitivity was examined in total of 59 strains from patients with cystic fibrosis and in 55 strains from control group. Examined pathogenicity factors of Staphylococcus aureus strains were - hemolysins, hyaluronidase and free coagulase production and biofilm formation. Hemolysins were examined on commercially available agar plate with sheep blood and on agar with washed sheep and human erythrocytes prepared in laboratory. Alfa-hemolysin, beta-hemolysin and delta-hemolysin were shown in strains isolated from patients with cystic fibrosis in 78,0%; 1,7% and 74,6%, respectively. From control group patients were alfa-hemolysin, beta-hemolysin and delta-hemolysin shown in 78,3%; 3,3% and 80,0%, respectively. In this evaluation was not demonstrated basic difference between strains of both groups. Free coagulase production was determined with commercial diagnostic set. Free coagulase from patients with cystic fibrosis was the most frequently positive in the second, the third and the fourth hour (66,1%; 10,2% and 10,2%, respectively) and from control group strains was free coagulase positive the most frequently in the second and third hour (55,0% and 31,7%, respectively). There was demonstrated similar biofilm formation between both groups of strains Staphylococcus aureus with regard to biofilm positive strains. Strains from patients with cystic fibrosis were biofilm positive in 77,2% and control group strains in 74,6%. However, strains from patients with cystic fibrosis were strongly biofilm positive in 31,6%, whereas control group strains were strongly positive only in 15,3%. There were more resistant Staphylococcus aureus strains from patients with cystic fibrosis (n = 41) to chosen antibiotics, especially to erythromycin (48,8%) and clindamycin (41,5%) in comparison with strains from control group (n = 48). Control group strains were much less resistant to erythromycin and clindamycin (23,6% and 20,0%,respectively). The exception was higher resistance to ciprofloxacin (30,9%) of control group strains in comparison with Staphylococcus aureus strains from patients with cystic fibrosis. There were 3,4% oxacillin resistant Staphylococcus aureus strains (MRSA) isolated from the total number of 59 Staphylococcus aureus strains isolated from patients with cystic fibrosis. There were 12,7% MRSA from the total number of 55 strains isolated from control group.
