Abstract:

Název: Léčebná rehabilitace po operaci šlachového aparátu ruky Dupuytrenova kontraktura Cíl: Hlavním cílem mé bakalářské práce je přiblížení problematiky Dupuytrenovy kontraktury, ucelení stěžejních informací o tomto onemocnění a přispění k řešení dané problematiky. Metodika: Pro své dva pacienty s Dupuytrenovou kontrakturou, kdy jeden byl léčen operativně, druhý konzervativně, jsem navrhla rehabilitační program. Z fyzioterapeutických metod jsem pro ošetření měkkých tkání a kloubů ruky používala myofasciální techniky a mobilizace dle Lewita. Pro zvětšení a udržení rozsahu pohybu jsem nejčastěji používala techniky dlahování, aktivní i pasivní pohyb, koncept vzpěrných cvičení dle Roswithy Brunkow a cvičení s Thera-Bandem. Kromě dlahování a pasivního pohybu jsem tytéž techniky využila i pro zvýšení svalové síly. V rámci redukce a prevence otoku byla použita elevace, cévní gymnastika, míčkování, masáž ruky a předloktí či kryoterapie. Výsledky: U prvního pacienta došlo ke zlepšení rozsahu pohybu do extenze při vstupním vyšetření byla omezena hybnost V. prstu, kdy rozsah aktivního pohybu do extenze byl v MP kloubu 65°, v PIP kloubu 80° a v DIP kloubu 45°. Při výstupním vyšetření dosáhl pacient plné extenze ve všech kloubech V. prstu. k dalšímu zlepšení u odoperované ruky došlo i v souvislosti s pretendinózními pruhy i uzlíky, které po operaci zcela zmizely. U druhého pacienta došlo též ke zlepšení rozsahu pohybu do extenze v MP kloubech obou rukou. U levé ruky se rozsah pohybu do extenze zvětšil u II. a III. prstu o 15°, u IV. prstu a V. prstu o 30°. U pravé ruky se zvětšila extenze u IV. prstu o 10° a u V. prstu o 25°. U obou pacientů došlo ke zlepšení schopnosti úchopů dle Kapandjiho. U neodoperovaných rukou se nám nepodařilo ovlivnit pretendinózní pruhy a uzlíky.

Abstract:

Title: Medical rehabilitation after hand operation the sinews apparatus of the hand Dupuytren's contracture Objective: The main aim of my thess is to approach the issue of Dupuytren's contracture, the completion of critical information about the disease and to contribute to solving of the issue. Methodology: For my two patients with Dupuytren's contracture, where one was treated operatively, the second conservatively, I drafted a rehabilitation program. For the treatment of soft tissue and joints of the hand used myofascial techniques and mobilization by Lewit. To enlarge and maintain range of motion, I frequently used splinting techniques, active and passive motion exercises according to the concept of buckling Roswitha Brunkow exercise and exercises with Thera-Band. In addition to splinting and passive movement, I also used the same techniques to increase muscle strength. Elevation, vascular training, balling, hand and forearm massage and cryotherapy were used as part of swelling's reduction and prevention. Results: The first patient improved range of motion in extension initial examination recorded limited momentum of 5th finger while the active range of motion in extension was 65° in the MP joint, 80° in PIP joint and 45° in DIP joint. Final examination of the client recorder a full extension in all joints the 5th finger. Further improvements of the operated hand's condition were connected with disposal of peritendinosis streaks and knots, which completely disappeared after the operation. Second patient's condition also improved, mainly the range of motion in extension of the MP joints in both hands. Motion range in extension improved in left-hand's condition, while 2nd and 3rd finger expanded by 15°, for 4th and 5th finger increment was 30°. For his right hand's motion range in extension the 4th finger improved by 10° and at the 5th finger by 25°. Both patients improved ability to carry out Kapandji grasps. We failed to affect unoperated hand's peritendinosis streaks and nods.
