The transition of the elderly from the medical facility to the natural social environment

Abstract:

Diplomová práce se zabývá problematikou návaznosti zdravotní a sociální péče pro seniory. Zaměřuje se na seniora závislého na pomoci druhé osoby v I. a II. stupni a na jeho přechod ze zdravotnického zařízení do přirozeného sociálního prostředí. Cílem této práce je zodpovědět hlavní výzkumnou otázku: „Jakým způsobem formální poskytovatelé pomoci ve vybrané lokalitě participují na přechodu seniora ze zdravotnického zařízení do přirozeného sociálního prostředí?“ Diplomová práce je rozložena do tří hlavních částí. V první části práce jsou vymezeny teoretické koncepty, které jsou východiskem pro další zkoumání. V metodologické části práce je popsána volba výzkumné strategie, kterou je kvalitativní strategie. Dále je objasněna operacionalizace dílčích výzkumných otázek, jednotka zkoumání a zjišťování, je popsána metoda sběru dat a realizace samotného výzkumu. Třetí, analytická část se zabývá rozborem a interpretací zjištěných skutečností. V závěru práce je podána odpověď na hlavní výzkumnou otázku.

Abstract:

This master thesis deals with the following health and social care issue for the elderly. Thesis is focused on elderly who are depend on the individual help in I. and II. degree and their transition from the medical facility to the natural social environment. The aim of this thesis is to answer the main question: "How formal help providers in the selected area participate in the transition of senior from medical facility to the natural social environment?" The thesis is divided into three main parts. In the first part are described theoretical concepts which are starting points for further investigation. The methodological part of the thesis describes the choice of research strategy, which is a qualitative strategy. Further the operationalisation of the research questions and units of the research are clarified. The second part of thesis is also focused on the data collection method and implementation of the research itself. The final part is analytical and deals with the analysis and interpretation of the findings. At the very conclusion is answered the main research question.
