The effect of non-pharmacological methods on the intensity of pain in children

Abstract:

Cíl: Cílem diplomové práce bylo zjistit efekt vybraných distrakčních metod ke snížení intenzity bolesti u dětských pacientů při procedurální bolesti. Pomocí systémové analýzy literárních zdrojů byly nalezeny nejvhodnější a nejúčelnější distrakční metody, které jsme v intervenční studii ověřili v praxi. Metodika: V praktické části byly systematickou analýzou literárních zdrojů nalezené vhodné distrakční metody, které prokázaly dobrý efekt při snižování intenzity bolesti u dětí. Nalezené distrakční metody byly testovány u vzorku 240 dětí ve věku 7-16 let, které podstoupily venepunkci v odborné ambulanci. Účinnost metod byla posouzená pomocí hodnocení bolesti při odběru krve na Wong-Bakerově obličejové škále bolesti, kterou hodnotilo samo dítě, rodič a odebírající sestra. Data byla sbírána v Nemocnici v Novém Jičíně a.s. na dětských odborných ambulancích v období duben-prosinec 2017. Výsledek: Výsledky intervenční studie potvrdily, že i v naších podmínkách jsou zvolené distrakční metody bez rozdílu účinné. Vhodným nenásilným a zábavným způsobem snížily vnímání intenzity bolesti u dětí při venepunkci. Dále jsme potvrdili, že bolest je vnímána bez rozdílu na pohlaví. Rozdílně je bolest vnímána u dětí mladších a starších. U části vzorků jsme nalezli ovlivnění intenzity bolesti přítomností rodiče u odběru. Závěr: Bolest je součástí nutných procedurálních výkonů a pomoci vhodných technik lze její intenzitu snížit. Tím jsme také schopni snížit distres dítěte a ovlivnit spolupracující vztah dítěte k zdravotnické péči. Veškeré testované distrakční metody jsou bez rozdílů účinné a dobře použitelné v praxi. V ideálním případě by měla být distrakční činnost součásti procesu odběru krve u dětí.

Abstract:

Objective: The aim of the diploma thesis was to find out the effect of selected distraction methods in pain intensity reduction on pediatric patients during procedural pain. Using systemic analysis we selected the most appropriate and most useful distraction methods that were verified in practice by intervention study. Methodology: In the practical part, a systematic analysis has found the suitable distraction methods, which have shown a good effect in decreasing the intensity of pain of children. The selected distraction methods were tested in a sample of 240 children aged 7-16 years who underwent venipuncture in a special outpatient clinic. The effectiveness of the methods was evaluated by assessing the pain using the Wong-Baker's blood-sampling pain range, which was evaluated by the child, the parents and the nurse taking care. The data was collected at the Hospital in Nový Jičín a.s. at children's outpatient clinics in April-December 2017. Result: The result of the intervention study confirmed that under out conditions the chosen distraction methods are indifferently effective. Using an appropriate, non-violent and entertaining way, they reduced the intensity of the pain in the children with venipuncture. We also confirmed that pain is perceived with no gender difference. Different pain is perceived in younger and older children. In some chidren, we found that parents presence influences the pain intensity either better or worse. Conclusion: Pain is a part of the necessary procedural procedures and can be reduced by using the appropriate techniques. By doing so, we are also able to reduce the child's distress and influence the child's cooperation to health care in the future. All tested distraction methods are effective and well-used in practice. Ideally, distraction should be an integral part of the blood collection process in children.
