PHYSIOTHERAPY FOR CHILDREN WITH TYPE 1 DIABETES MELLITUS (T1DM)

Abstract:

Zpracovala jsem kazuistiku 14letého probanda s diagnózou DM1. V rámci vstupního vyšetření jsem z aplikace Nightscout zjistila měsíční profil glykemií sledovaného probanda a tzv. time in range (čas strávený v cílových hodnotách). Provedla jsem vstupní kineziologický rozbor, který zahrnoval antropometrické vyšetření, vyšetření funkční svalové síly, zkrácených svalových skupin, hypermobility dle prof. Jandy a vyšetření HSS. Na základě výsledků jsem stanovila krátkodobý plán fyzioterapie, který byl sestaven z 14 terapií v průběhu 3 měsíců. První část terapie obsahovala řízenou pohybovou aktivitu zaměřenou na aerobní cvičení střední zátěže v délce 20 - 40 minut. Druhá část terapie byla zaměřena na úpravu postavení hlezenního kloubu, plochonoží a svalových dysbalancí, aktivaci HSS pomocí metody SMS a Brügger konceptu. Během terapie jsem monitorovala pomocí systému kontinuálního monitorování glykemie Dexcom G6 probandovu glykémii. Na konci fyzioterapeutické intervence byl proveden výstupní kineziologický rozbor, na jehož základě byl probandovi navržen dlouhodobý fyzioterapeutický plán s ohledem na DM1.

Abstract:

I processed the case report of a 14-year-old proband diagnosed with DM1. As a part of the initial examination, I used the Nightscout app to obtain the monthly glycemia profile of the monitored proband and the time in range (time spent in target values). I performed an initial kinesiology analysis, which included an anthropometric examination, examination of functional muscle strength, shortened muscle groups, hypermobility according to prof. Janda and HSS examination. Based on the results, I established a short-term plan of physiotherapy, which consisted of 14 therapies over the course of 3 months. The first part of the therapy consisted of regulated physical activity focused on medium-intensity aerobic exercise lasting 20 40 minutes. The second part of the therapy was focused on adjusting the position of the hock joint, flat feet, activation of the HSS using the SMS method and the Brügger concept. During the therapy I monitored the proband's blood glucose using the Dexcom G6 continuous blood glucose monitoring system (CGM). At the end of the physiotherapy intervention, an output kinesiological analysis was performed, on the basis of which a long-term physiotherapy plan was suggested to the proband with regard to DM1.
