Red blood cell antibodies investigation in imunohematology laboratory

Abstract:

Vyšetření protilátek proti erytrocytům tvoří základ práce v imunohematologických laboratořích. Protilátky jsou vyšetřovány při předtransfuzním vyšetření, u gravidních žen, u osob s hematologickými chorobami, u potransfuzních reakcí a u dárců krve. V první části mé práce se zaměřuji na teoretické poznatky, na základní imunohematologické pojmy, na vztahy mezi antigenem a protilátkou. V další části podávám přehled hlavních skupinových systémů na erytrocytech člověka a možných protilátek. V praktické části se zabývám jednotlivými laboratorními postupy vyšetření protilátek na svém pracovišti – Transfuzním oddělení Fakultní nemocnice Královské Vinohrady v Praze, s přihlédnutím k nejnovějším poznatkům a doporučením Společnosti pro transfuzní lékařství ČLS JEP. V experimentální části své práce uvádím statistický přehled vyšetření protilátek proti erytrocytům v imunohematologické laboratoři TO FNKV za celý rok 2011 a vyhodnocuji výsledky vyšetření protilátek vzhledem k teoretickým poznatkům. Na konci práce popisuji tři zajímavé kazuistiky vyšetření protilátek.

Abstract:

Red blood cell antibody investigation forms the basis of work in imunohematology laboratory. The antibodies are investigated during pre-transfusion examinations and post-transfusion reaction examinations, and also during medical examinations of pregnant women, persons with haematological diseases, and blood donors. In the first part of my work I focus on theoretical knowledge, on essential terms in immunohematology and the relationship between an antigen and an antibody. The next section provides an overview of human blood groups and possible red blood cells antibodies. The practical part deals with individual laboratory antibody testing procedures at my workplace - The Blood Transfusion department of the University Hospital Royal Vinohrady in Prague, in the light of latest findings and recommendations of the Society for Transfusion Medicine ČLS JEP. In the experimental part of my work I provide a statistical overview of red blood cell antibody investigations undertaken in the immunohematology laboratory at my workplace during the year 2011 and evaluate the results of antibody testing compared with theoretical knowledge. At the end of my work I describe three interesting casuistry of antibody investigations.
