Kangaroo mother care at preterm infants

Abstract:

Cíl: Cílem výzkumu bylo zjistit, zda provádění sezení klokánkování u nedonošených novorozenců v průběhu hospitalizace na JIRPN FNO, bude mít u těchto novorozenců vliv na jejich srdeční a dechovou frekvenci, periferní saturaci krve kyslíkem, délku hospitalizace, poměr mezi množstvím přijaté umělé stravy a mateřského mléka. Dalším cílem bylo zjistit, zda klokánkování ovlivní interakci matka-dítě. Metodika: Výzkumný soubor tvořilo 33 nedonošených novorozenců a jejich matek, Podmínkou pro zařazení byla porodní hmotnost 1800 gramů a méně a stabilizovaný zdravotní stav. Technikou sběru dat k posouzení efektu klokánkování bylo měření fyziologických funkcí a analýza zdravotnické dokumentace. Interakce mezi matkami a jejich dětmi byly měřeny dotazníkem - Maternal Postnatal Attachment Scale (MPAS). Ke statistickému zpracování byl využit t-test pro dva výběry, neparametrický Wilcoxonův test, chí test a Fisherův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Z měřených fyziologických funkcí byl pouze signifikantně potvrzen vzestup tělesné teploty. Při porovnání intervenční a kontrolní skupiny před klokánkováním nebyl v interakcích matek k jejich dětem zaznamenán žádný signifikantní rozdíl. Před propuštěním (resp. po klokánkování) byl zájem o dítě u matek klokánkovaných dětí významně vyšší (p = 0, 0354) oproti matkám, které své dítě neklokánkovaly. Ve všech položkách MPAS byla úroveň připoutání ke klokánkovanému dítěti před propuštěním vyšší než u neklokánkovaných novorozenců. Závěr: Klokánkování prokázalo pozitivní efekt na nedonošené novorozence v oblasti vzestupu tělesné teploty, zvýšeného zájmu o potřeby novorozence a silnějšího připoutání matky k novorozenci.

Abstract:

Aim: The aim of research was to find out if kangarooing at premature newborns during hospitalisation at the NICU of University hospital in Ostrava has at these newborns influence on their heart and breathing frequency, suburban saturation of blood by oxygen, length of hospitalization and relation between the amount of accepted artificial food and breast milk. Another aim was to find out if kangarooing influences the interaction mother-child. Methods: The researching aggregate was made of 33 premature newborns and their mothers. The condition for including into the research was the birth weight of 1800 grams and lower and stabilised state of health. The method of collecting the data to access the effect of kangarooing was measuring of physiological functions and analysis of medical documentation, the interactions between mothers and the children were measured by questionnaire Maternal Postnatal Attachment Scale (MPAS). For statistical processing was used t-test for two selections, nonparametric test of Wilcox, chí test and exact test of Fisher. The statistical tests were valued of the level of importance 5 %. Results: From the measured physiological functions only increasing of body temperature was significantly confirmed. When comparing intervention and check group before kangarooing there was not any significant difference in interactions. Before discharging from hospital (after kangarooing) the interest in children was enormously higher at mothers of kangarooed children in comparison with mothers who did not kangaroo. In all items of MPAS the level of attachment to kangarooed children before discharging from hospital was higher than at children who were not kangarooed. Conclusion: Kangarooing showed positive effect on premature newborns in the field of increasing body temperature and interest in needs of newborns and stronger attachment of mother to her newborn baby.
