Abstract:

ŠLACHY ROTÁTOROVÉ MANŽETY RAMENNÍHO KLOUBU - ANATOMICKÉ ASPEKTY A METODY LÉČBY Cílem klinické části projektu bylo zkoumání účinku nových léčebných postupů u pacientů se syndromem bolestivého ramena. Cílem experimentální části práce byla studie přítomnosti akcesorního svalu musculus subscapularis. Klinická část studie byla rozdělena do tří podčástí. Do první části studie byli zařazeni pacienti s impingement syndromem ramena. U 25 pacientů byly aplikovány 3 injekce plasmy bohaté na trombocyty (PRP) do subakromiálního prostoru, kontrolní soubor 25 pacientů byl ošetřen jednorázovou injekcí depotního kortikoidu. Vyšetření pacientů probíhalo bezprostředně před léčbou, 6 týdnů, 3 měsíce a 6 měsíců od aplikace. Ve druhé studii byl hodnocen ukazatel rizika reruptury šlachy musculus supraspinatus- tangenciální znamení. Ve třetí studii byl na 25 pacientech s rupturou šlachy musculus supraspinatus zkoumán efekt peroperačního užití mezenchymálních kmenových buněk (MSCs) na hojení a incidenci reruptury šlachy. V experimentální části jsme provedli anatomickou pitvu obou ramenních kloubů na 25 kadaverozních preparátech. V případě, že byl nalezen akcesorní sval pod spodním okrajem musculus subscapularis, zkoumali jsme detailně anatomii tohoto svalu a jeho vztah k musculus subscapularis. V klinické části práce se podařila prokázat efektivita biologických léčebných postupů u pacientů se syndromem bolestivého ramena, stejně tak byl prokázán přínos předoperačního hodnocení tangenciálního znamení pro predikci reruptury šlachy musculus supraspinatus. Ve skupině 50 vyšetřených ramen na kadaverozních preparátech byl nalezen akcesorní sval ve 42 případech. Jestliže byl sval nalezen na jedné straně, byl vždy nalezen i na druhostranném ramenním kloubu téhož kadaveru. Výsledky práce ukazují, že použití biologických postupů u pacientů se syndromem bolestivého ramena poskytuje slibné výsledky i ve srovnání s pacienty, kteří byli ošetřeni standardními metodami ošetření. Rovněž je možné potvrdit, že akcesorní sval musculus subscapularis může být považován za samostatný sval, který by mohl plnit funkci antagonisty musculus supraspinatus.

Abstract:

TENDONS OF THE ROTATOR CUFF MUSCLES OF THE SHOULDER JOINT – ANATOMICAL ASPECTS AND METHODS OF TREATMENT The aim of the clinical part of the project was to investigate the impact of new treatment methods on patients with painful shoulder syndrome. The aim of the experimental part was to study the presence of the accessory muscle of the subscapularis muscle. The clinical part of the study was divided into three sub-sections. The first part of the study included patients with the shoulder impingement syndrome. In 25 patients, 3 injections of platelet rich plasma (PRP) were injected into the subacromial space, a control group of 25 patients was treated with a single injection of depot corticosteroid. Patients were examined immediately before the treatment, 6 weeks, 3 months, and 6 months after the application. In the second study, the risk indicator for the rerupture of the supraspinatus muscle tendon - tangent sign- was evaluated. In the third study, the effect of peroperative use of mesenchymal stem cells (MSCs) on healing and incidence of tendon rerupture on 25 patients with rupture of the supraspinatus muscle tendon was evaluated. In the experimental section we performed anatomical dissection of both shoulder joints on 25 cadavers. If the accessory muscle was found under the lower margin of the subscapularis muscle, we examined in detail the anatomy of this muscle and its relation to the subscapularis muscle. In the clinical part of this work, the effectiveness of biological treatment methods in patients with painful shoulder syndrome was demonstrated, as well as the benefit of the preoperative evaluation of the tangential sign for the prediction of the supraspinatus muscle tendon rerupture. In a group of 50 dissected cadaveric shoulders, an accessory muscle was found in 42 cases. If the muscle was found on one side, it was always found on the contralateral shoulder joint of the same cadaver. The results of the work show that the use of biological procedures in patients with painful shoulder syndrome provides promising results when compared with patients treated with standard treatment methods. It is also possible to confirm that the musculus subscapularis accessory muscle can be a separate muscle that could serve as a supraspinatus muscle antagonist.
