Neinvazivní biomarkery karcinomu ledviny jako možný nástroj pro screening a časnou diagnostiku

Non-invasive biomarkers of renal cancer as a possible tool for screening and early detection

Abstract:

V současné době není kromě zobrazovacích vyšetření dostupná žádná jiná diagnostická metoda pro časnou detekci karcinomu z renálních buněk. Stejně tak neexistuje biomarker použitelný v praxi pro diagnostiku karcinomu z renálních buněk nebo screening populace. Cílem dizertační práce je přehled aktuálních diagnostických možností pro neinvazivní časnou detekci karcinomu z renálních buněk a ve výzkumné části identifikace diagnostických mikroRNA (miRNA) v moči pacientů s expanzivním procesem na ledvině umožňujících časný záchyt karcinomu z renálních buněk. Do studie bylo zařazeno celkově 105 pacientů (69 pacientů s histologicky verifikovaným světlobuněčným karcinomem z renálních buněk v preparátu po radikální nefrektomii nebo resekci ledviny a 36 kontrol). U všech subjektů byla odebraná první ranní moč, u pacientů s nádorem před jeho chirurgickým odstraněním. Metodou kvantitativní Real-Time PCR array byly v moči stanoveny expresní profily miRNA z rodiny let-7 a jejich hodnoty byly srovnány mezi skupinou pacientů s nádorem a zdravými kontrolami. Výsledky byly statisticky vyhodnoceny. Ve skupině pacientů s nádorem byla ve srovnání s kontrolami v moči prokázána statisticky signifikantně vyšší koncentrace všech studovaných let-7 miRNA. Nejlepších výsledků pro odlišení nemocných a zdravých subjektů bylo dosaženo u let-7a (senzitivita 71%, specificita 81%, AUC 0,83). U ostatních let-7 byla analytická přesnost nedostačující (AUC ˂ 0,75). Zvýšená exprese mikroRNA z rodiny let-7 v moči pacientů s renálním karcinomem umožňuje použití těchto mikroRNA jako možných diagnostických biomarkerů pro časnou diagnostiku karcinomu z renálních buněk. Slibných výsledků bylo dosaženo zejména u let-7a. Tyto výsledky je potřeba potvrdit v dalších studiích. Dle našeho vědomí se jedná o jednu z prvních studii týkajících se močových miRNA v diagnostice renálního karcinomu.

Abstract:

There is currently no diagnostic modality for the early detection of renal cell carcinoma other than radiological studies. At the same time there is no existing biomarker to diagnose renal cell carcinoma and enable population screening. The aim of the dissertation is to review current diagnostic methods for noninvasive detection of renal cell carcinoma and in experimental part identification of diagnostic microRNAs (miRNAs) in urine of patients with renal mass enabling the early detection of renal cell carcinoma. 105 subjects were enroled in the study (69 patients with histologically confirmed clear-cell renal cell carcinoma after radical nephrectomy or partial nephrectomy and 36 controls). First voided morning urine samples were obtained (prior to surgical removal of the tumor in the patients group). Urinary expression profiles of let-7 miRNA family members were analyzed using the Real-Time PCR array and compared between the two study groups. Statistical analysis was performed. Urinary concentration of all studied let-7 miRNAs was statistically significantly higher in the patients group compared to healthy controls The best results for discrimination between the patients group and healthy controls were obtained with let-7a (sensitivity 71%, specificity 81%, AUC 0,83). The rest of let-7 miRNAs showed unsatisfactory analytical performace (AUC ˂ 0,75). Increased urinary expression levels of let-7 miRNAs indicate their accuracy as noninvasive diagnostic biomarkers for the early detection of renal cell carcinoma. The results of let-7a are promising and should be confirmed with further studies. To our knowledge, our study is one the first studies evaluating urinary miRNAs in the diagnosis of renal cell carcinoma.
