Rehabilitation of an elderly patient with Parkinson's Disease in advanced stage: case report

Abstract:

Parkinson's disease (PD) is a complex neurodegenerative disorder with consequences for patients and their families. The degeneration of ~ 60% of the dopaminergic neurons of the substantia nigra represents the first sign of desease. This degeneration causes motor dysfunction with four symptoms- postural instability, bradykinesia, muscular rigidity and resting tremors. Nonmotor symptoms may also occur and precede motor dysfunction by several years. Management of PD has been conventionally centered on drug therapy, with levodopa viewed as the "gold standard" treatment. However, an optimal medical management support the inclusion of rehabilitation therapies, numerous studies investigating the e?cacy of rehabilitation therapies in patients with PD and have demonstrated the bene?cial e?ect of rehabilitation therapies on motor and swallowing function, activities of daily living, and quality of life.

Abstract:

La malattia di Parkinson ? un disturbo neurologico degenerativo che presenta gravi ripercussioni per il paziente e i suoi familiari. I primi sintomi clinici si manifestano solo dopo che il 60% circa dei neuroni dopaminergici della substantia nigra risulta gi? degenerato. Tale degenerazione produce disfunzioni che si manifestano con quattro sintomi primari - tremori a riposo, bradicinesia, rigidit? muscolare e instabilit? posturale. Sintomi non motori possono manifestarci anche molti anni prima. Il trattamento del Morbo di Parkinson viene comunemente realizzato impiegando il farmaco levodopa, inquadrato come il trattamento principale. In ogni caso, un approccio ottimale presuppone l'inclusione di terapie riabilitative, numerosi studi infatti hanno evidenziato l'efficacia di tali trattamenti in pazienti affetti da Morbo di Parkinson, e hanno dimostrato gli effetti positivi sulle funzioni motrici, le attivit? giornaliere e la qualit? della vita.
