An Analysis of Nursing Diagnoses for Patients after Hip Replacement Surgery

Abstract:

Cíl: Cílem bakalářské práce je analyzovat ošetřovatelské diagnózy u pacientů po totální endoprotéze kyčelního kloubu na základě analýzy ošetřovatelské dokumentace. Dalším cílem je srovnat výskyt ošetřovatelských diagnóz u pacientů po totální endoprotéze kyčelního kloubu s komplikacemi a bez komplikací a dále zjistit, jaké ošetřovatelské intervence byly zaznamenány u jednotlivých ošetřovatelských diagnóz. Metodika: Průzkumný soubor tvořila dokumentace 129 pacientů hospitalizovaných v nemocnici v Novém Jičíně, u kterých byla provedena operace totální endoprotézy kyčelního kloubu v roce 2011. Na základě analýzy zdravotnické a ošetřovatelské dokumentace byly vyhodnoceny ošetřovatelské diagnózy a intervence použité u těchto pacientů. Výsledky: Analyzováno bylo celkem 16 ošetřovatelských diagnóz, které tvoří soubor ošetřovatelských diagnóz používaných na daném oddělení. Vyhodnocením získaných dat bylo zjištěno, že nejčetnějšími ošetřovatelskými diagnózami byly: Bolest, Porucha soběstačnosti a sebepéče a Omezená pohyblivost. U aktuálních ani u potencionální ošetřovatelských diagnóz nebyly zaznamenány významné rozdíly ve vedení ošetřovatelské péče u pacientů s komplikacemi nebo bez komplikací. U aktuálních ošetřovatelských diagnóz byla nejčetnější intervence: aplikace analgetika dle ordinace lékaře a sledování jejich účinku, u potencionálních ošetřovatelských diagnóz to byly dvě intervence se stejnou četností: sledovat krvácivé projevy, monitorovat pooperační průběh a vznik možných komplikací. Závěr: Bakalářská práce rozšiřuje poznatky o souboru ošetřovatelských diagnóz a intervencí u pacientů po totální endoprotéze kyčelního kloubu. Dále poskytuje náhled na používání ošetřovatelské diagnostiky u sester v klinické praxi.

Abstract:

Aim: The thesis analyzes the nursing diagnoses of patients after total hip surgery using nursing documentation. Another aim of the thesis is to compare and analyze the occurrences of the nursing diagnoses prescribed to patients who during their ambulation a) experienced medical complications, b) did not experience medical complications. The thesis further describes which nursing interventions were recorded in response to individual nursing diagnoses. Methodology: The selected sample is composed from 129 records of the patients, who were hospitalized in year 2011, in Nový Jičín Hospital after undergoing total hip surgery. Based on these records was created an analysis (a) of the nursing diagnoses and (b) of the interventions related to these patients. Results: Sixteen nursing diagnoses were analyzed closely, which are being currently recorded at the above-mentioned Hospital department. Evaluated data shows the most frequent nursing diagnoses: pain, failure of self?sufficiency and self?care and movement limitations. Actual nursing diagnosis showed differences which were recorded to patients both with and without medical complications. These two batches of patients did not have any differences in their potential nursing diagnosis. The most frequent actual nursing diagnosis was: Application of analgesics according to the doctor´s instructions and the supervising of the effects of this application. The most frequent potential nursing diagnoses were (with the same number of occurrences): Supervising the bleeding symptoms, post?operation monitoring of the patient´s ambulation and the formation of possible complications. Conclusion: The results of the thesis extend information about the set of the nursing diagnostics and interventions related to patients after total hip surgery. The thesis provides an aid to the recording of nursing diagnostics for nurses in clinical practice.
