Immune mechanisms, diagnosis and treatment of food allergy

Abstract:

Potravinová alergie je nežádoucí stav imunitního systému reagující na běžně se vyskytující alergeny v naší potravě projevující se různou intenzitou a závažností zasaženého orgánového systému. Tato imunologická odezva mnohdy bývá zaměňována za jinou nepříznivou reakci na potraviny, a proto je nezbytné ji vyspecifikovat od intolerancí, pseudoalergií či dokonce autoimunitního onemocnění. Dle svého imunologického účinku se jedná o reakci zprostředkovanou protilátkami typu IgE a/nebo buněčnými mechanismy, tzv. non IgE mediovanou odpověď. Prevalence alergií, včetně potravinových stoupá a během posledních let bylo seskupeno množství nových informací a poznatků týkajících se epidemiologie s vlivem genetických a environmentálních faktorů na vznik a rozvoj tohoto onemocnění. Současné metody léčby nejsou zcela optimální, spočívají zejména v eliminaci daného alergenu ze stravy a v podpůrné léčbě zmírňující příznaky, proto je v této oblasti velký potenciál pro implementaci vhodné terapie, s příkladem experimentální léčbou imunoterapií. Cílem této práce je shrnout současné poznatky týkající se epidemiologické situace, nahlédnout do imunologického mechanismu, tedy patogeneze samotného onemocnění, a alergenů jej způsobující, diagnózy a v neposlední řadě možnosti léčby jakožto terapeutických inovací perspektivních do budoucna.

Abstract:

Food allergy is an adverse condition of the immune system responding to common allergens in our diet, manifested by varying intensity and severity of the affected organ system. This immune response is often mistaken for another adverse reaction to food, and therefore it is necessary to specify it from intolerances, pseudoallergies or even autoimmune disease. Depending on its immunological mechanisms, it is a reaction mediated by IgE-type antibodies and/ or cellular mechanisms, the so-called delayed non-IgE-mediated response. The prevalence of allergies, including food allergies, is rising, and in recent years a wealth of new information and knowledge has been gathered regarding epidemiology with the influence of genetic and environmental factors on the origin and development of this disease. Current methods of treatment are not entirely optimal, they consist mainly in the elimination of the allergen from the diet and in supportive treatment to alleviate symptoms, so there is great potential in this area for the implementation of appropriate therapy, with an example of experimental treatment with immunotherapy. The goals of this work is to summarize the current knowledge about the epidemiological situation, to look into the immunological mechanism, ie the pathogenesis of the disease itself, and allergens causing it, diagnosis and, last but not least, treatment options as therapeutic innovations promising for the future.
