Medical rehabilitation after fractures of the proximal end of the femur

Abstract:

Cíl: Cílem práce je řešení problematiky s využitím fyzioterapeutických postupů, metod a konceptů k rychlejší a úspěšnější rekonvalescenci pacienta. Metodika: Sestavila jsem pacientce rehabilitační plán, který jí bude dle jejího věku vyhovovat. Po operaci jsem ihned využila cévní gymnastiku, míčkovou facilitaci a polohování pro snížení otoku a prevenci před tromboembolickou nemocí. Velmi důležité je hojení jizvy, kam jsem zařadila mazání jizvy a tlakovou masáž. Dalšími metodami jsou kondiční cvičení, aktivní i pasivní pohyby, které jsou prováděny na zvýšení rozsahu pohybu v kloubu a udržení mobility. Pro posílení oslabených svalů jsem využila práci s thera bandem, overballem , PNF i RS. Použila jsem také Brügger koncept k správnému nastavení držení těla a korekci sedu a pro podpoření napřímení páteře akrální koaktivační terapii. S pacientkou jsem cvičila hluboký stabilizační systém vleže na zádech, využívala jsem zde i Metodu Ludmily Mojžíšové pro posílení pánevního dna. U cvičení jsem okrajově použila i metodu spiral dynamic. V neposlední řadě jsme nacvičovali sebeobsluhu a vertikalizaci do sedu, stoje, chůze ve vysokém a později v nízkém chodítku. Výsledky: Po výstupním fyzioterapeutickém vyšetření jsem zjistila, že měřené hodnoty v porovnání se vstupním vyšetřením se zlepšily. Důležité změny při porovnání obvodů, hybnosti, síly operované DK u vstupního a výstupního vyšetření: obvod 15 cm nad patellou z 46 cm na 41 cm, obvod přes koleno z 39 cm na 37 cm, aktivní a pasivní hybnost u kyčle do flexe: z 30/35° na 55/60°, do abdukce: z 15/15° na 30/35°. Extenzi, addukci a rotace jsem při vstupním vyšetření nevyšetřovala z důvodu režimových opatření nebo neschopnosti pacientky přetočit se na břicho. Extenzi v kyčelním kloubu jsem vyšetřila až při výstupním vyšetření, mohu tedy rozsahy pohybu porovnat pouze se zdravou DK: operovaná 5° a zdravá 15°. Orientační svalová síla se u flexe zlepšila z 3 na 4 a u abdukce z 2 na 3+. Pacientka nyní provádí abdukci v kyčelním klubu fyziologicky bez tensorového mechanismu. Závěr: Využitím komplexní fyzioterapeutické péče jsem u pacientky dokázala zvýšit rozsah pohybu, svalovou sílu, zmírnit bolest operované končetiny, posílit svaly a naučit pacientku správným pohybovým stereotypům. Pacientce jsem doporučila cvičit a provádět aktivity dle dlouhodobého fyzioterapeutického plánu.

Abstract:

The objective: The main goal of this thesis is a solution of the given problem by using physiotherapy techniques, methods and concepts for faster and more successful recovery of the client. Methodology: I have assembled an age-suitable rehabilitation plan for the patient. After the operation, I immediately applied the vascular gymnastics, mick facilitation and positioning to reduce swelling and prevent thromboembolic disease. Very important is the healing of scars, where I placed lubrication of the scars and pressure massage. Other methods are fitness exercises, active and passive movements, which are used to increase the range of motion in the joint and maintaining of the mobility. I used to work with thera band, overball, PNF and RS to strengthen weak muscles. I also used Brügger concept for the correct posture and correct sitting position. The acral coactivation therapy is used for supporting the spine strightening. I used the method of Ludmila Mojžíšová to strengthen the pelvic muscles and deep stabilization system. For exercise, I also involved spiral dynamic methods. Finally, we practiced self-care and verticalization in sitting, standing, walking in high and later in the low walker. Results: After the exit of physiotherapy examination, I compared the measured values. The important changes with comparing circuits, momentum, strenght operated leg at the entrance and exit examinations circumference: 15 cm above the patella: 46-41 cm circumference knee 39-37 cm, active and passive momentum in hip flexion: 30/35° - 55/60° in abduction: 15/15 ° - 30 / 35 °. Extension, abduction and rotation at entrance examination, I did not investigate because the patient had these moves prohibited or she could not turn to her belly. I investigated the extension of the hip joint when I finished the examination. I can compare only the range of motion with healthy DK: operated 5/15° healthy. Tentative muscle strength at flexion improved from 3 to 4 and for the abduction from 2 to 3+. The patient does abduction without a tensor mechanism. Conclusion: I proved the increase of motion, muscle strenght, pain reduction of the operated leg and I taught the patient to do the correct motion stereotypes by using the comprehensive physiotherapy. I recommended her to exercise and do the physiotherapy activities during the long-physiotherapy plan.
