Abstract:

ABSTRAKT STOLIČKA, Milan. Paliativní péče u pacientů s demencí [disertační práce]. Ostravská univerzita v Ostravě, Lékařská fakulta, Ústav ošetřovatelství a porodní asistence. Školitel: PhDr. Radka Bužgová, Ph.D. Studijní obor: P5341 Ošetřovatelství. Stupeň odborné kvalifikace: doktor filozofie (philosophiae doctor, Ph.D.). Úvod: Stárnutí populace patří k významným demografickým jevům a stává se významným celosvětovým problém dnešní doby. Kvalita paliativní péče o pacienty s těžkým stupněm demence a podpora jejich pečovatelů-rodinných příslušníků představují jeden z významných problémů současné doby. Cíl: Cílem výzkumu bylo zjistit, jaké jsou nejčastější negativní symptomy u pacientů s těžkým stupněm demence, jestli dochází k ovlivnění jejich výskytu a jestli dochází ke změně kvality jejich života v průběhu hospitalizace na lůžku následné péče (LDN). Dalším cílem bylo zjistit jaké je kvalita života pečovatelů-rodinných příslušníků těchto pacientů, jaké jsou jejich nejdůležitější potřeby a jak jsou naplněny. Metodika: Do souboru bylo zařazeno 105 pacientů s pokročilou demencí hospitalizovaných v LDN Klokočov a 87 rodinných příslušníků. Pro sběr dat u pacientů s demencí byly zvoleny tyto hodnotící nástroje: škála kvality života v pozdní fázi demence QUALID (Quality of life in late-stage dementia scale), SM-EOLD (Symptom Management at the End-of-Life in Dementia), který je určen ke sledování symptomů u pacienta s pokročilou demencí a CILQ (Cognitively Impaired Life Quality Scale - CILQ) pro vnímání kvality života pacientů s kognitivní poruchou. U pečovatelů-rodinných příslušníků byly použity hodnotící nástroje QOLLTI-F (Quality of Life in Life Threatening Illness - Family Carer Version) pro hodnocení kvality života rodinných příslušníků a FIN (Family Inventory of Needs) ? měření potřeb rodinných příslušníků. Výsledky a diskuze: V rámci výzkumu bylo zjištěno, že nejčastějšími obtížemi pacientů s těžkou demencí jsou bolest, dušnost, deprese a úzkost. Tyto symptomy a kvalita života těchto pacientů byly v rámci institucionální péče pozitivně ovlivněny. U pečovatelů-rodinných příslušníků dochází ke změnám v oblasti životních podmínek, stavu, příbuzenských vztahů a v oblasti financí. Převažují změny pozitivní. Prioritními potřebami jsou základní informace a komfort pacienta. Naplněnosti většiny potřeb nebylo dosaženo. Závěr: v rámci paliativní péče o pacienty s demencí lze ovlivnit obtěžující symptomy a zlepšit kvalitu jejich života. U pečovatelů-rodinných příslušníků nejsou dostatečně naplněny jejich potřeby.

Introduction: The aging population is an important demographic phenomena and becomes an important global issue of our time. The quality of palliative care for patients with severe dementia and support for their caregivers-family members represent one of the major problems of our time. Aim: The aim of the research was to find out what are the most common negative symptoms in patients with severe dementia, if there is an influence on their occurrence and whether there is a change in the quality of life during hospitalization in Long-term care institution. Methods: The group included 105 patients with advanced dementia hospitalized in Long-term care institution Klokocov and 87 family members. For data collection in patients with dementia were chosen following evaluation tools: a range of quality of life in late stage dementia QUALID (Quality of life in late-stage dementia scale), SM-EOLD (Symptom Management at the End-of-Life in Dementia) which is designed to monitor a patient with symptoms of dementia advanced CILQ (Cognitively impaired life quality Scale - CILQ) for perception of the quality of life for patients with cognitive impairment. For caregivers-family members were used assessment tools QOLLTI-F (Quality of Life in Life Threatening Illness - Family Carer Version) for assessing the quality of life of family members and FIN (Family Inventory of Needs) ? measurement of needs of family members. Results and discussion: The survey found that the most common problems in patients with severe dementia are pain, shortness of breath, depression and anxiety. These symptoms and quality of life of these patients were in institutional care positively affected. For caregivers family members, changes in living conditions, marital status, family relationships and in finance. Outweigh the positive changes. Priority needs are basic information and patient comfort. Fulfilling the needs of the majority was not achieved. Conclusion: In the context of palliative care for patients with dementia bothersome symptoms can be affected and their quality of life can be improved. For caregivers-family members is not possible to meet their needs.
