Physiotherapy in patient with parkinson´s disease

Abstract:

Úvod: Parkinsonova nemoc je neurodegenerativní onemocnění, projevující se motorickými i non-motorickými symptomy. Léčba zahrnuje farmakoterapii, chirurgické zákroky i fyzioterapii. Cílem léčby je zmírnit symptomy a zlepšit kvalitu života pacientů. Prognóza je individuální a závisí na faktorech jako je celkové zdraví a reakce na léčbu. Práce se zaměřuje na popis teoretických poznatků odborné literatury ve vztahu k danému onemocnění a na konkrétní průběh fyzioterapeutické intervence u diagnostikovaného pacienta. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta s Parkinsonovou nemocí. Metodika: Popis teoretických poznatků obsahuje vybrané rešerše a analýzu odborné literatury. Speciální část obsahuje kazuistiku pacienta s Parkinsonovou nemocí . Součástí je anamnéza, odebrána formou rozhovoru a vstupní kineziologický rozbor. Jeho obsahem je goniometrické a antropometrické měření, vyšetření aspekcí, palpací a kloubní vůle. Dále funkční svalový test dle prof. Jandy a neurologické vyšetření. Na základě kineziologického rozboru je sestaven krátkodobý fyzioterapeutický plán. Terapie probíhaly 1krát týdně po dobu 45-60 minut, v průběhu osmi týdnů. Používané fyzioterapeutické metody a koncepty: TMT, PFS, PIR, mobilizační techniky, aktivní cvičení proti odporu s využitím TheraBandu, senzomotorická stimulace, PNF a ACT a autoterapie. V závěru je proveden výstupní kineziologický rozbor a následné porovnání vstupních a výstupních hodnot kineziologických rozborů. Dále je vytvořen návrh dlouhodobého fyzioterapeutického plánu. Výsledky: Výsledky této práce poukazují na posílení původně oslabených svalů v oblasti hýždí a obecně svalů pánevní arei, na zlepšenou protažitelnost zkrácených prsních svalů a hamstringů, minimalizaci kloubních blokád, navýšení rozsahu pohybu kyčle, kolene a ramenního kloubu, a v neposlední řadě poukazují na významné ovlivnění stability a stereotypu chůze. Závěr: Na základě svědomitého přístupu pacienta, dodržování krátkodobého fyzioterapeutického plánu, vzájemné spolupráce a domácí autoterapie, došlo k zlepšení motorických funkcí, celkové kondice, a také k zlepšení psychického stavu pacienta.

Abstract:

Introduction: Parkinson's disease is a neurodegenerative disorder characterized by motor and non-motor symptoms. Treatment includes pharmacotherapy, surgery, and physiotherapy. The aim of treatment is to relieve symptoms and improve the quality of patient's life. Prognosis is individual and depends on factors such as overall health and treatment response. In the theoretical part Parkinson's disease is described including current research findings. In the experimental (practical) part, the course of physiotherapeutic interventions with a diagnosed patient are presented. The objective: The main goal of the thesis is to contribute to solving the problems of physiotherapy in a patient with Parkinson´s disease. Methodology: The review of theoretical information includes selected research and an analysis of literature. The specific section presents a case study of a patient with Parkinson's disease. It comprised of a medical history obtained during an interview and an initial kinesiological analysis. This analysis includes goniometric and anthropometric measurements, observation, palpation, joint play, muscle function testing by prof. Janda and neurological examination. Based on the kinesiological analysis, a short-term physiotherapy plan was defined. The course of physiotherapeutic intervention is outlined, 45-60 minutes individual sessions took place once a week for the duration of eight weeks. Utilized physiotherapeutic methods and concepts include softtissue techniques, post-facilitation stretching, mobilisation techniques, active resistance exercise using TheraBand, sensorimotor stimulation, proprioceptive neuromuscular facilitation and acral coactivation therapy and autotherapy. The conclusion includes final kinesiological analysis and a subsequent comparison of initial and final data with a proposed long-term physiotherapy plan. Results: The results of this thesis point to the strengthening of the gluteal muscles and the muscles of the pelvic area, improved extensibility of the pectoral muscles and hamstrings, the reduction of joint blockages, the increase in the motion range of the hip, knee and shoulder joint, And to the significant influence on the stability and stereotype of walking. Conclusions: Adherence to a short-term physiotherapy plan and mutual cooperation led to an improvement in motor functions, physical condition and psychological well-being of the patient.
