Delirium in patients with acute stroke

Abstract:

Práce poskytuje komplexní pohled na delirium u pacientů s akutní cévní mozkovou příhodou a u pacientů po operačních zákrocích. Hodnotí incidenci deliria, prognózu, predisponující a vyvolávající rizikové faktory. Vytvořili jsme českou verzi testu The Confusion Assessment Method for the Intensive Care Unit (CAM-ICUcz), screeningového testu určeného pro detekci deliria u pacientů v akutní péči a provedli jsme jeho validaci. Potvrdili jsme vysokou incidenci deliria u pacientů s akutním iktem, dále jsme prokázali prodlouženou dobu hospitalizace, horší funkční stav a vyšší mortalitu u pacientů s deliriem. Nejvýznamnější rizikové faktory deliria byli věk a preexistující kognitivní deficit. Byla prokázána asociace metabolických abnormalit s výskytem deliria. Studie srovnávající incidenci deliria u pacientů s akutní cévní mozkovou příhodou a u pacientů po operačním zákroku prokázala významně vyšší incidenci deliria u pacientů s primárním poškozením mozku. Vytvořili jsme dva alternativní prediktivní statistické modely deliria u pacientů s akutní cévní mozkovou příhodou, jejichž využití umožňuje zavést účinné preventivní opatření vedoucí ke zlepšení výsledného stavu pacientů po akutním iktu.

Abstract:

Study provide a comprehensive view of delirium in patients with acute ischemic stroke and patients after surgical intervention. We evaluated the incidence of delirium, prognosis, predisposing and precipitating risk factors. We have authorized a Czech version of the test The Confusion Assessment Method for the Intensive Care Unit (CAM-ICUcz), a screening tool designed to detect delirium in patients in acute care, and made its validation. We confirmed the high incidence of delirium in patients with acute stroke, prolonged hospitalization, poorer functional status and higher mortality in patients with delirium. The most important risk factors for delirium are age and preexisting cognitive deficit. We have demonstrated the association of metabolic abnormalities and delirium. Study comparing the incidence of delirium in patients with acute stroke and patients after surgery has brought the result of a significantly higher incidence of delirium in patients with primary brain damage. We have created two alternative predictive statistical models of delirium in patients with acute ischemic stroke, the use of which allows the introduction of effective preventive measures to improve the outcome of patients after acute stroke.
