Establishing a psychotherapeutic relationship with adolescents, in which the therapist identified attachment disorder

Abstract:

ABSTRAKT Navázaní psychoterapeutického vztahu s dospívajícím s poruchou attachmentu je důležitý úkol psychote-rapeuta, neboť vztah v psychoterapii pomáhá. Dospívající nedovršili plnoletosti, a proto do terapie vstupují rodiče. Práce se tedy zaměřuje na to, jak spolupráce s rodiči ovlivňuje navazování psychoterapeutického vztahu s dospívajícím s poruchou attachmentu. V literatuře jsou dospívající zahrnováni do dětské psycho-terapie a jejich specifika nejsou často zdůrazňována. Dosud chybí (nebo jsem nenalezla) studii, která by od-pověděla na to, jak spolupracovat s rodiči a přitom vytvářet psychoterapeutický vztah s dospívajícím s poruchou attachmentu. S cílem prozkoumat tuto otázku byl navržen kvalitativní výzkum, který v rozhovorech se čtyřmi respondenty o osmi kazuistikách mapoval, jakou mají terapeuti zkušenost s navazováním vztahu za uvedených podmínek. Ke sběru dat byl použit polostrukturovaný rozhovor a k analýze byla zvolena metoda Interpretativní fenomenologické analýzy (IPA). Výsledky analýzy před-kládají hlavní zkušenosti terapeutů a klíčová témata jednotlivých rozhovorů – Individualizace terapie dle klientovy potřeby zapojit rodiče, Ostražitost a odtažitost – projevy dospívajícího ve vztahu, Vztah ke klientům jako zdroj nejistoty a potřeby opatrnosti, Terapeut jako dosycující jistá attachmentová osoba a Přirozené ovlivnění vztahu díky pohlaví terapeuta. Cílem práce je nalézt a zmapovat zkušenosti terapeutů s navazováním terapeutického vztahu a zjistit, jak se do této iniciální fáze promítá porucha attachmentu dospívajících a spolupráce s rodiči. Povědomí o specifických postupech a vhodném způsobu navázání terapeutického vztahu s dospívajícími s poruchou attachmentu a postup spolupráce s rodiči by mohly terapeutům následně usnadnit navázání vztahu. Smyslem práce je zároveň přispět výzkumem do oblasti psychoterapie dospívajících.

Abstract:

ABSTRACT Establishing the psychotherapeutic relationship with adolescents who have the disorder of the attachment is an important task for psychotherapist because the relationship in psychotherapy helps. Teenagers are not adults yet therefore parents participate in the therapy. The thesis is focused on collaboration with parents which affects the establishment of the psychotherapeutic relationship with adolescents who have the disor-der of the attachment. There are no specific information about teenagers in child psychotherapy literature. No study is written (or I wasn't able to find it) about cooperation with parents and establishing psychother-apeutic relationship with that adolescent. Qualitative research, which contained interviews with four re-spondents about eight casuistries, was designed to analyze it. The main topic of the research is the therapist's experience with establishing the relationship with defined conditions. Data were collected using semi-structured interviews and for the analysis was chosen interpretative phenomenological analysis (IPA). The results of the analysis present major experiences of therapists and main topics of the interviews – Indi-vidualization of therapy according to clients' needs to involve parents, Precaution and detachment - symptoms of a teenage relative, Relationship with clients as a source of uncertainty and the need for caution, Therapist as a secure attachment figure, Natural affecting the relationship because of sex therapist. The aim of this thesis is to identify and define the experiences of therapists in establishing a therapeutic relationship and figure out how the disorder of the attachment of adolescents and cooperation with parents affect the initial phase. Awareness of specific procedures and the appropriate way how to establish a therapeutic relationship with adolescents with the disorder of the attachment and cooperating with parents could help therapists to make establishing the relationship easier. Another purpose of this work is to extend current research of psychotherapy of adoles-cents.
