Physiotherapy in patients with headaches

Abstract:

Úvod: V této bakalářské práci se zabývám problematikou fyzioterapie u pacientů trpících bolestí hlavy. Cíl práce: Cílem práce je přispět k léčbě bolesti hlavy, a to především k řešení problematiky volby adekvátních fyzioterapeutických postupů, konceptů a metod. Metodika práce: V hlavní části práce se zabývám kazuistikou 21leté pacientky trpící bolestmi hlavy. Nejdříve jsem zhotovila vstupní kineziologický rozbor, jehož součástí byla anamnéza, antropometrie, goniometrie, svalové testy, testy zkrácených struktur a pohybových stereotypů dle prof. Jandy (2004), testy hluboké stabilizace trupu dle prof. Koláře (2020) a testy neurologické. Pro stanovení intenzity bolesti jsem využila vizuální analogovou škálu bolesti. Na základě zjištěných informací jsem stanovila krátkodobý plán fyzioterapie. V průběhu 3 měsíců proběhlo celkem 10 terapií. Následně byl vypracován výstupní kineziologický rozbor, na jehož základě jsem vytvořila dlouhodobý fyzioterapeutický plán. Výsledky: U pacientky došlo k obnovení rozsahu pohybu krční páteře a k obnově správného dechového stereotypu a trupové stabilizace. Dále se nám podařilo posílit oslabené svaly a protáhnout svaly zkrácené. Pacientka uvádí subjektivní snížení intenzity a frekvence bolestí hlavy. Závěr: Jelikož byla pacientka snaživá a motivovaná, pracovalo se mi s ní velice dobře. Díky její svědomitosti a práci i mimo terapie jsme docílili požadovaných výsledků. Proto doufám, že tato práce přispěje ke změně přístupu k terapii s pacienty, kteří mají stejný problém.

Abstract:

Introduction: In this bachelor's thesis, I deal with the issue of physiotherapy for patients suffering from headache. The objective: The objective of the thesis is to contribute to the treatment of headaches, and above all to the solution of the issue of choosing adequate physiotherapeutic procedures, concepts and methods.. Methodology: In the main part of the thesis, I deal with the case study of a 21-year-old female patient suffering from headaches. First, I compiled an initial kinesiology analysis, which included anamnesis, anthropometry, goniometry, muscle tests, tests of shortened structures and movement stereotypes according to prof. Jandy (2004), tests of deep stabilization of the trunk according to prof. Wheels (2020) and neurological tests. I used a visual analogue pain scale to determine the intensity of ache. Based on the information assembled, I established a short-term physiotherapy plan. A total of 10 therapies took place over the 3-months course. Subsequently, an output kinesiology analysis was prepared, on the basis of which I established a long-term physiotherapy plan. Results: The range of motion of the cervical spine and the correct breathing pattern and trunk stabilization were restored to patient. Furthermore, we managed to strengthen weakened muscles and stretch shortened muscles. The patient reports a subjective reduction of intensity and frequency of headaches. Conclusions: Since the patient was hardworking and motivated, the course took well cooperation. Thanks to her conscientiousness and extra practice even among therapies, we achieved the desired results. Therefore, I hope that this thesis will contribute to changing the approach to therapy with patients who suffer the same problem.
