Oral health in children with orthodontic appliances

Abstract:

Cíl práce: Cílem diplomové práce bylo zjistit úroveň orálního zdraví a rozsah znalostí o ústní hygieně dětí léčených ortodontickými aparáty. Dále zaznamenat změny v orálním zdraví a úrovni znalostí v průběhu léčby rozdílnými typy ortodontických aparátů. Metodika: Ke sběru dat, byl použit standardizovaný dotazník WHO - Oral Health Questionnaire for Children a záznamová karta k vyšetření dítěte. Výzkumný soubor tvořilo 200 dětí ve věku 6-18 let. Pro popis souboru byla použita popisná statistika. Pro vyhodnocení hypotéz byl použit chí-kvadrát test a Fisherův exaktní test. Výsledky: Šetřením se ukázalo, že zubní plak v dutině ústní se častěji vyskytoval u skupiny dětí bez rovnátek. Doba léčby má vliv na přítomnost plaku v dutině ústní u dětí s rovnátky, čím déle rovnátko nosí, tím více plaku se objevuje. Byl zjištěn vztah mezi délkou léčby ortodontickým aparátem a stavem dásní, pomocí PBI indexu, který nám sloužil jako ukazatel stavu dásní. Neprokázalo se, že by hodnoty PBI indexu měly souvislost s různými typy ortodontických aparátů. Výsledky dotazníkové studie prokázaly lepší rozsah znalostí o ústní hygieně u skupiny dětí léčených ortodontickými aparáty. Závěr: Ze srovnání výsledků vyšetření zkoumaných skupin pacientů vyplývá, že lepší úroveň orálního zdraví měly děti s ortodontickými aparáty než kontrolní skupina dětí bez rovnátek. Jelikož se prokázalo, že u ortodontické léčby probíhající déle než jeden rok dochází k zhoršení orální hygieny, je nutné pacienty opakovaně motivovat k pravidelným návštěvám zubního lékaře a edukovat je o správné dentální hygieně i v průběhu léčby.

Abstract:

Objective: The aim of the thesis was to determine the level of oral health and the extent of knowledge about oral hygiene of children receiving orthodontic appliances. Furthermore, experience changes in oral health and knowledge level during treatment with different types of orthodontic appliances. Methods: Data were collected using a standardized questionnaire WHO Oral Health Questionnaire for Children and record card to the examination of the child. The sample consisted of 200 children aged 6-18 years. For a description of the file was used descriptive statistics. To evaluate the hypotheses using the chi-square test and Fisher's exact test. Results: The investigation showed that plaque in the mouth was more frequent in the group of children without braces. The duration of treatment is influenced by the presence of plaque in the oral cavity in children with braces, longer wearing the braces worn by more plaque appears. Relationship was found between duration of treatment orthodontic appliance and the gums, using PBI index, which served us as an indicator of the gums. There is no evidence that PBI index values were associated with different types of orthodontic appliances. The results of the questionnaire study demonstrated improved range of knowledge about oral hygiene in a group of children receiving orthodontic appliances. Conclusion: A comparison of the results of the tests examined groups of patients suggests that a better level of oral health had children with orthodontic appliances than the control group of children without braces. Since it has been demonstrated that for orthodontic treatment in progress for more than a year there is a deterioration in oral hygiene, it is necessary to re-motivate patients to regular visits to the dentist and to educate them about proper dental hygiene and during treatment.
