Depression in the elderly with rheumatoid arthritis

Abstract:

Cíl: Cílem práce bylo zjistit souvislost deprese s omezením funkčních schopností pacienta, stupněm bolesti a stádiem onemocnění revmatoidní artritidou u seniorů. Metodika: Výzkum měl charakter kvantitativního šetření. Sběr dat byl proveden pomocí standardizovaného dotazníku HAQ (Health Assesment Questionare), deprese pomocí dotazníku BDI II (Beckova samoposuzovací škála deprese) Bolest pomocí vizuální analogové škály (VAS). Aktivitu onemocnění charakterizoval index DAS28. Dále byly použity demografické data, věk a pohlaví. Výzkumný soubor tvořilo 228 respondentů. Testovali jsme pomocí Pearsonova chí-kvadrát testu, hladina významnosti byla 0,05. Výsledky: Byl zjištěn 46% výskyt deprese, a to v kategoriích mírné, střední a těžké deprese. Minimální příznaky deprese vykazovalo 54 % respondentů. Výsledky výzkumu prokázaly, že vyšší omezení funkčních schopností způsobuje vyšší výskyt depresí (p=0,0027). Mezi bolestí a výskytem deprese byla rovněž zjištěna významná závislost (p = 0,0329) Čím vyšší hodnota bolesti VAS, tím vyšší míra deprese. Výzkum neprokázal souvislost mezi depresí a aktivitou onemocnění (p = 0,71437). Závěr: Z výsledků usuzujeme, že deprese se při onemocnění RA vyskytuje ve vysokém zastoupení. Stupeň deprese pak ovlivňuje stupeň omezení funkčních schopností a bolest. Na stupeň deprese nemá vliv aktivita onemocnění.

Abstract:

Aim: The aim of the thesis was to determine the association of depression with the limitation of the patient's functional abilities, the degree of pain and the stage of rheumatoid arthritis in the elderly. Methods: Quantitative research methods were used. Data were collected by stan-dardized questionnaire HAQ (Health. Assessment Questionnaire), rate of depression was evaluated by BDI II (Beck's Depression Inventory II). The pain was marked on the visual analogue scale (VAS). The disease activity was characterized by the DAS28 index. Demographics, age, and gender data were also used. The research sample consisted of 228 respon?dents. We tested with Pearson's chi-square test, the significance level was 0.05. Results: In the research, the rate of depression was found 46 % in the categories of mild, moderate and severe depression. Minimal symptoms of depression reported 54 % of respondents. The results of the research have shown that a higher limitation of functional abilities causes a higher incidence of depression (p = 0.0027). There was also a significant dependence between pain and the incidence of depression (p = 0.0329) The higher the VAS pain value, the higher the rate of depression. Research has not shown a link between depression and disease activity (p = 0.71437). Conclusion: From the results we concluded that depression occurs in diseases of rheumatoid arthritis in high representation. The limit of depression affects the degree functional limitations abilities and pain. Depression does not affect the activity of the disease.
