Nové prognostické skóre pro hodnocení přežití a odpovědi na léčbu u pacientů s chronickou myeloidní leukémií

New prognostic score for the evaluation of survival and treatment response in patients with chronic myeloid leukaemia

Abstract:

Vývoj cílené léčby v posledních letech výrazně prodloužil život pacientů s chronickou myeloidní leukémií, a se změnou léčebných postupů a výsledků tak začaly být dosavadní nástroje pro odhad prognózy pacientů nedostačující. Práce je zaměřena na návrh nového prognostického skóre, které by umožnilo rozdělit pacienty léčené cílenou léčbou (konkrétně imatinibem) podle potenciálního rizika hematologické progrese na základě krevních parametrů zjištěných při diagnóze. S použitím Coxova regresního modelu a metody frakčních polynomů byla na souboru 319 pacientů z českého registru INFINITY vytvořena dvě skóre – FP skóre a S skóre. U rizikových skupin podle obou skóre je porovnáváno přežití pacientů do hematologické progrese, ale i celkové přežití, přežití do akcelerované fáze či blastického zvratu a dosažení odpovědí na léčbu. Obě skóre jsou v práci srovnávána s obecně používanými Sokalovým, Hasfordovým a EUTOS skóre i s nejnovějším ELTS skóre. Výsledky jsou rovněž ověřovány na validačním souboru 180 pacientů z českého registru CAMELIA.

Abstract:

Developments in targeted therapy have considerably prolonged the life expectancy of patients with chronic myeloid leukaemia. As the therapy standards and results has changed, the existing tools for estimating the prognosis of patients has become insufficient. The aim of the study is to design a new scoring system to distinguish patients treated with targeted therapy (imatinib) according to their risk of haematological progression based on blood parameters acquired at diagnosis. Using the Cox regression model and methods of fractional polynomials, two prognostic scores were developed using the data of 319 patients from the Czech INFINITY registry – the so-called FP score and the S score. A comparison of the risk groups defined by the scores is performed with regard to the progression-free survival until haematological progression as well as the overall survival, the progression-free survival until accelerated phase or blast crisis and achievement of treatment responses. Besides, both scores are compared with widely used Sokal, Hasford, EUTOS and recent ELTS scores. The results are also externally validated on 180 patients from the Czech CAMELIA registry.
