The contribution of the navigation system in the surgical treatment of trauma from the radiographer´s point of view.

Abstract:

ÚVOD: Popisuji historii a princip počítačem řízených navigačních systémů v medicíně. Podrobněji pak rozebírám komponenty a software navigačního systému používaného na Traumatologickém centru FN Ostrava. Objasňuji možnosti získávání podkladových dat pro tyto systémy. Popisuji anatomii a druhy zlomenin u femuru a pánve. Uvádím možnosti operační léčby různými druhy osteosyntéz. CÍL: Stanovit počty zlomenin vhodné k navigačnímu operačnímu řešení v průběhu let 2008 a 2009. U operovaných pacientů sledovat skiaskopické časy, dávky na DAP metru RTG přístroje a operační časy. Sledovat úlohu radiologického asistenta při navigovaných operačních výkonech. Na základě dotazníkového šetření zjistit názory odborné veřejnosti na navigační systém v traumatologii. METODIKA: Retrospektivně vytvářím dva soubory pacientů, u kterých sleduji skiaskopické časy, dávky a operační časy. Zjišťuji názory na navigační systém u odborné veřejnosti. VÝSLEDKY: Pro operační řešení s využitím navigačního systému je vhodných 8% u femuru a 24% u pánve. Skiaskopický čas u navigovaných výkonů je kratší, dávky menší. Úloha radiologického asistenta se mění. Odborná veřejnost je velmi dobře informována. ZÁVĚR: Zjišťuji, že navigační operační výkony mají kratší skiaskopické časy a menší radiační zátěž. U pánve je operační čas kratší u navigovaných operačních výkonů. Náplň práce radiologického asistenta na operačním sále se při použití navigačního systému mění.

Abstract:

INTRODUCTION: I describe the history and principles of computer-controlled navigation systems in medicine. I analyze in a more detail the components and software of the navigation system used at the Traumatology Centre of the University Hospital Ostrava. I specify the options for obtaining the underlying data for these systems. I describe the anatomy and various types of femoral and pelvic fractures. I list the treatment options with different types of osteosyntheses. AIM: To define the number of fractures suitable for navigation surgical treatment during the period of 2008 and 2009. To monitor the skiascopic times, doses at DAP metres and the instrument, as well as the operating times in patients undergoing surgery. To observe the role of radiographers in the course of navigated surgical procedures. Using a questionnaire, to determine the opinion of the specialists upon the use of navigation systems. METHODS: Retrospectively, I create two groups of patients where I monitor the skiascopic times, radiation doses and operating times. I monitor the opinion of specialists concerning the navigation systems. RESULTS: The surgical treatment with the use of a navigation system is suitable for 8 per cent of femoral and 24 per cent of pelvic fractures. The skiascopic time is shorter and the doses in the navigated procedures are lower. The radiographer?s role changes. Specialists are very well informed about the topic. CONCLUSION: I conclude that the navigated surgical procedures present shorter skiascopic times and a lower radiation burden. The operating time in pelvic fractures in shorter when using the navigation system. The role of radiographer at the operating theatre during navigated procedures changes.
