Molecular genetic diagnostics of neuromuscular diseases

Abstract:

Ve své práci jsem se zaměřila na molekulárně genetickou diagnostiku Duchennovy a Beckerovy svalové dystrofie, facioskapulohumerální svalové dystrofie a pletencové svalové dystrofie. Duchennova a Beckerova svalová dystrofie (DMD/BMD) jsou spojeny s mutacemi v genu DMD (Xp21.2). Jedná se o typicky X vázaný přenos: postiženi jsou muži, ženy jsou většinou přenašečky bez klinického projevu onemocnění. V rámci doktorské práce byla molekulárně genetická diagnostika cílena na detekci bodových mutací a malých delecí/inzercí. V našem souboru pacientů jsme identifikovali 78 různých mutací malého rozsahu, z nich 46 dosud nebylo v literatuře popsáno. Popsali jsme 3 pacienty s mutací, která vytvářela předčasný terminační kodon [p.(Glu55*), p.(Glu1110*), p.(Tyr1274*)], a dalo by se tedy očekávat, že pacienti budou mít fenotyp DMD, zjištěný fenotyp ale odpovídal mírnější variantě, tedy BMD. U všech 3 pacientů jsme na úrovni mRNA nebo na úrovni proteinu identifikovali pravděpodobný mechanismus asociovaný se změnou těžkého fenotypu na lehký. U jednoho pacienta jsme popsali neobvyklou kombinaci dvou dědičných neuromuskulárních onemocnění, DMD a onemocnění Charcot-Marie-Tooth. Facioskapulohumerální svalová dystrofie (FSHD) je onemocnění s velmi variabilním klinickým projevem, včetně velké intrafamiliární variability. Dědičnost FSHD je dominantní a patogeneze tohoto onemocnění je odlišná od většiny monogenních chorob. FSHD je způsobena snížením počtu repetic D4Z4 v subtelomerní oblasti chromozomu 4 (v 95 %, FSHD1) nebo mutacemi v genu SMCHD1 (FSHD2). Obě situace vedou k rozvolnění chromatinové struktury v oblasti repetic a následné transkripci mRNA genu DUX4 ve svalech. Zavedli jsme molekulárně genetickou diagnostiku FSHD. V případě FSHD1 se jedná o určení velikosti fragmentu nesoucího repetice D4Z4, který je u FSHD1 pacientů 10–40 kb (což odpovídá 1–10 repeticím). U FSHD2 byla zavedena mutační analýza genu SMCHD1. FSHD1 jsme prokázali u 187 probandů. Korelací dostupných klinických dat s počtem repetic jsme potvrdili, že pacienti s malým počtem repetic (1–3 D4Z4) mají nižší věk nástupu onemocnění a těžší klinický průběh vedoucí až ke ztrátě schopnosti samostatné chůze. Mutace v genu SMCHD1 byla zatím nalezena u jednoho pacienta. Recesivní pletencové svalové dystrofie (LGMD2) jsou geneticky heterogenní skupina s velmi podobnými klinickými projevy. V současnosti je popsáno 21 genů v souvislosti s LGMD2 a právě molekulární diagnostika umožňuje konečné potvrzení typu LGMD2. Mutační analýzou genů CAPN3, FKRP, SGCA a ANO5 jsme určili frekvenci nejčastějších typů LGMD2 v české populaci. Četnost zastoupení detekovaných forem je 32,6 % pro LGMD2A, 4,1 % pro LGMD2I, 2,8 % pro LGMD2D a 1,4 % pro LGMD2L. Použitím metody sequence capture a targeted resequencing jsme detekovali další dva typy LGMD2, tj. LGMD2B a LGMD2E.

Abstract:

In this PhD thesis I focused on molecular genetic diagnostics of Duchenne and Becker muscular dystrophy, facioscapulohumeral muscular dystrophy and limb-girdle muscular dystrophy. Duchenne and Becker muscular dystrophy (DMD/BMD) is associated with mutations in the DMD gene (Xp21.2). The inheritance is typical X-linked: males are affected; females are carriers without clinical symptoms. The aim of molecular genetic diagnostics was the detection of point mutations and small deletions/insertions in the DMD gene. In our group of patients we identified 78 different mutations, 46 of them have not been published yet. We found 3 patients with mutations creating premature termination codon [p.(Glu55*), p.(Glu1110*), p.(Tyr1274*)], patients were supposed to develop severe phenotype DMD, but the phenotype was milder BMD. We described probable mechanism of rescuing the DMD phenotype in these three cases. We report one patient with an unusual combination of two inherited neuromuscular disorders – DMD and Charcot-Marie-Tooth disease. Facioscapulohumeral muscular dystrophy (FSHD) is disorder with variable clinical symptoms including high intrafamiliar variability. FSHD is inherited in an autosomal dominant manner and the pathogenesis is unlike most monogenic disorders. FSHD is caused by contractions of the D4Z4 repeat array on the chromosome 4 (95 %, FSHD1) or by mutations in the SMCHD1 gene (FSHD2). Both situations lead to a partial opening of the D4Z4 chromatin structure and transcription of D4Z4-encoded DUX4 mRNA in muscles. We introduced molecular genetic diagnostics of FSHD. FSHD1 is confirmed by detection of shortened fragment carrying the repeat array, the length of fragment is 10–40 kb (what corresponds with 1–10 repeats). In the case of FSHD2 patients, the mutation analysis of the SMCHD1 gene was performed. FSHD1 was confirmed in 187 probands. We correlated the available clinical data with detected number of repeats and found out that small number of repeats is connected with earlier onset and more severe phenotype. We detected a mutation in the SMCHD1 gene in the case of one patient. Autosomal recessive limb-girdle muscular dystrophies (LGMD2) are a highly heterogeneous group of muscle disorders on genetic level but with similar phenotype. Up to date 21 genes are connected with LGMD2 and only molecular diagnostics leads to finally confirmation of LGMD2 type. We determined the frequency of LGMD2 subtypes within a cohort of Czech LGMD2 patients using mutational analysis of the CAPN3, FKRP, SGCA, and ANO5 genes. The frequency of particular forms of LGMD2 was 32.6 % for LGMD2A, 4.1 % for LGMD2I, 2.8 % for LGMD2D, and 1.4 % for LGMD2L. Using method sequence capture and targeted resequencing, we identified patients with other two rare forms, LGMD2B and LGMD2E.
