Alcohol in Czechia: Regional disparities and determinants

Abstract:

Cílem práce je zmapovat a prozkoumat regionální disparity úmrtnosti související s alkoholem na úrovni okresů ČR. Tyto disparity se dle dostupných studií mohou významně podílet na celkových úmrtnostních nerovnostech a zároveň mohou poukazovat na specifická zdravotní rizika v daných regionech a vysvětlit jejich příčiny. Zdrojem dat použitých v práci je neveřejná regionální databáze KROK, která obsahuje strukturované údaje za kraje a okresy a databáze zemřelých dle příčin úmrtí se zaměřením na tzv. alkoholové příčiny (cirhóza, alkoholismus, otravy alkoholem). Analyticky je cíl práce řešen v první fázi redukcí dimenzionality proměnných pomocí faktorové analýzy. Metrikou úmrtnosti jsou věkově standardizované míry. Regionální podobnosti jsou sledovány pomocí shlukové a korelační analýzy. Souvislost mezi faktory prostředí a alkoholovou úmrtnosti je pak odhadována vícerozměrnou lineární regresí; analýza pokrývá období 2016-2018. Úmrtnost související s alkoholem je systematicky vyšší ve východní části země, u obou pohlaví je pak mírně zvýšená i v okresech severočeského pohraničí. U mužů je regionální diferenciace alkoholové úmrtnosti podobná jako u celkové úmrtnosti, u žen jsou tyto vazby méně patrné. V obecné regionální diferenciaci okresů lze identifikovat skryté faktory, které poukazují na faktory chování (např. rozvodovost), na makroekonomické faktory (struktura zaměstnanosti) a na velikostní strukturu sídel, přičemž strukturní faktory jsou dle analýz významnější. Analýzy neprokázaly existenci zásadního determinujícího faktoru, naopak se zdá, že problematika úmrtnosti související s alkoholem je široce podmíněna celou řadou faktorů, z nichž některé (jako například tradice nebo rozsah a charakter místních sociálních sítí) jsou s dostupnými daty těžko měřitelné.

Abstract:

The objective of this work is to map and look into regional disparities of mortality related to alcohol consumption at the Czechia district level. According to available studies, these disparities can significantly contribute to total mortality inequality and can also point at specific health risks in their respective regions as well as explain their causes. The source of the data in this work is the non-public regional database KROK which contains structured data for regions and districts, as well as a database containing the data of deaths by different causes with a focus on so-called “alcohol causes” (cirrhosis, alcoholism, alcohol poisoning). Analytically, the aim of the work is dealt with in the first phase by reducing the dimensionality of the variables using factor analysis. Age-standardized measures are the metric of mortality. Regional similarities are monitored using cluster and correlation analysis. The connection between the environmental factors and alcohol mortality is calculated using multiple linear regression, the analysis covers years 2016-2018. Alcohol-related mortality is systematically higher in the east of the country, it is also slightly higher in the northern border regions. For the male population, the regional differentiation of alcohol-related mortality is similar to overall mortality, while those ties are less apparent in the female population. Underlying factors can be identified in the general regional differentiation of the districts, and those factors can be tied to behavior factors (e.g., divorce rate), macroeconomic factors (e.g., the structure of employment), and also to the size structure of residences, while the structural factors are more significant, according to the analyses. The analyses did not prove the existence of an essential determining factor, it seems, however, that the question of alcohol-related mortality is widely conditional and based on an array of factors. Some of those factors, such as customs or the range and character of local social networks, are difficult to measure with available data.
