Využití natriuretických peptidů u srdečního selhání a infarktu myokardu

Application of natriuretic peptides in heart failure and myocardial infarction

Abstract:

Srdeční selhání spolu s infarktem myokardu jsou nejčastější kardiologické diagnózy vedoucí k hospitalizaci a také diagnózy s vysokým podílem na mortalitě u globální populace. Tato dizertační práce je tvořená třemi částmi. První část je teoretickým úvodem do problematiky srdečního selhání, infarktu myokardu a natriuretických peptidů. Druhá a třetí část je tvořená výsledky naších klinických výzkumů. Části textu dizertační práce byly publikovány jako přehledné články, jejichž je autor prvním nebo korespondenčním autorem. Srdeční selhání je primárně diagnostikováno u více jak 1 milionu hospitalizaci ročně. Přežívání pacientů s diagnostikovaným srdečním selháním se v průběhu posledních let významně zlepšuje, přesto je u těchto pacientů asi 50% mortalita do 5 let po stanovení diagnózy. Mortalita u pacientu s infarktem myokardu je až 1,8 milionu úmrtí ročně, neboli 20 % všech úmrtí v Evropě. Natriuretické peptidy (NP) patří mezi intenzivně využívané biomarkery v různých oblastech klinické praxe a jsou předmětem mnoha studii. Mají nezastupitelnou úlohu v diagnostice, vedení terapie a rizikové stratifikaci nejen u pacientů se srdečním selháním ale i u pacientů s infarktem myokardu a v řadě dalších klinických jednotek. V spojení s klinickými skórovacími systémy mají ještě lepší výsledky. Praktická část této disertační práce prezentuje výsledky k stratifikaci STEMI pacientů vhodných k časné dimisi pomoci natriuretických peptidů a Zwolle skóre. Na základě současných doporučení za nízce rizikové pacienty, schopné časné dimise, jsou považování ti se Zwolle skóre ≤ 3 body. Z výsledků naší studie vyplývá, že kombinaci Zwolle skórovacího systému spolu s BNP (cut-off ≤ 200 pg/ml) je možno navýšit počet bezpečně propustitelných nízko-rizikových pacientů až o cca 50%. Časné propuštění těchto nízce rizikových pacientů po STEMI může snížit riziko jejich nozokomiálních infekcí a snížit náklady na léčbu. V druhé části praktické práce jsou prezentovány výsledky sledování natriuretických peptidů u stabilních pacientů 3 měsíce po STEMI léčených primární PCI, sloužící k identifikaci nejrizikovějších pacientů. U těchto pacientů zvýšené hodnoty BNP≥189 pg/ml a NT-proBNP≥551 pg/ml predikují zvýšené riziko hospitalizace pro srdeční selhání, a kombinovaného klinického end-pointu (hospitalizace pro srdeční selhání a mortalita) během prvních dvou let. Určení hodnot NP s odstupem 3 měsíců po STEMI lépe odráží rozvoj remodelace a srdečního selhání s nutnosti zavedení agresivnější terapie a důslednějších kontrol pacienta.

Abstract:

Heart failure and myocardial infarction are the most common cardiological diagnoses leading to hospitalization as well as diagnosis with a high mortality rate in global population. This dissertation consists of three main parts. The first part deals with a theoretical introduction to the problems of heart failure, myocardial infarction and natriuretic peptides. The second and third part contains results of our clinical research. Text sections from the dissertation have been published as articles. Heart failure is primarily diagnosed with more than 1 million hospitalizations per year. Survival of patients with heart failure has improved significantly in recent years. However, mortality rate of these patients is still about 50% in following 5 years after making diagnosis. Mortality due to myocardial infarction is up to 1.8 million deaths per year, otherwise 20% of all deaths in Europe. Natriuretic peptides are intensively used biomarkers in various areas of clinical practice and clinical studies. They have an irreplaceable role in diagnosis, managing therapy and risk stratification in wide spectrum of cardiological and non-cardiological diseases. In conjunction with clinical scoring systems they have got even better results. Practical part of the dissertation thesis presents results for the stratification of STEMI patients suitable for early hospital discharge using natriuretic peptides and Zwolle score. According to current recommendations for low-risk patients, they are considered those with Zwolle score ≤ 3 points. Based on the outcomes of our study, the combination of the Zwolle scoring system and BNP (cut-off ≤ 200 pg / ml) we can increase the number of safely low-risk patients by up to 50%. Early discharge of low-risk STEMI patients may reduce the risk of their nosocomial infections and reduce the cost of treatment. In second part of the practical work there are presented the results of monitoring of natriuretic peptides in stable patients 3 months after STEMI, treated by primary PCI, helping to identify the most risky patients. Considering these patients, elevated BNP ≥ 189 pg / ml and NT-proBNP ≥551 pg / ml predict an increased risk of hospitalization for heart failure and a combined clinical endpoint (hospitalization for heart failure and mortality) during the first two years. Determining NP values 3 months after STEMI, better reflects the development of remodelling and heart failure with the need for more aggressive therapy and more rigorous patient control.
