Shoulder impingement syndrome treatment by Dr. Kirsch´s method

Abstract:

Práce popisuje metodu Dr. Johna Kirsche a možnosti jejího využití v terapii a léčbě impingement syndromu. Vzhledem k tomu, že se v České republice jedná o metodu téměř neznámou, pokusili jsme se tuto metodu blíže popsat, ozřejmit a aplikovat v praxi. V úvodů práce je popsán fylogenetický vývoj horních končetin, vybrané poznatky z kineziologie ramenního pletence a scapulohumerálního rytmu a bližší informace týkající se problematiky impingement syndromu. V poslední kapitole teoretické části jsou uvedeny poznatky o metodě Dr. Kirsche, její aplikaci, a jejím možnému využití v terapii impingement syndromu. Praktická část zkoumá skupinu 8 probandů s diagnózou impingement syndrom, kteří se po vstupním vyšetření zúčastnili 3měsíční studie vlivu aplikace Kirschovy metody na snížení bolesti měřené pomocí numerické škály bolesti (NRS). Výzkumná skupina probandů byla nejdříve podrobena vstupnímu vyšetření a následně zainstruována k pravidelnému domácímu cvičení. V průběhu šetření byli probandi studie v měsíčních intervalech 3měsíčního testovacího období dotazováni na změny intenzity bolesti a tyto hodnoty byly následně vyhodnoceny. Výsledkem této práce bylo zjištění, zda je tato metoda vhodná při léčbě impingement syndromu a zda má její aplikace pozitivní vliv na snížení bolesti oproti vstupním naměřeným hodnotám NRS.

Abstract:

This bachelor thesis describes the method of John Kirsch MD and possibilities of its use in therapy and treatment of impingement syndrome. Since this method is almost unknown in the Czech Republic, we have tried to describe, clarify and apply this method in practice. Phylogenetic development of upper extremities, selected findings from kinesiology of shoulder girdle and scapulohumeral rhythm and more information on impingement syndrome are described in the introduction part of this theses. The last chapter of the theoretical part presents the knowledge about the Dr. Kirsch´s method, its application as well as its potential use in impingement syndrome therapy. The practical part examines a group of 8 probands with the diagnosis of impingement syndrome who participated in the 3month long study of the effect of Kirsch's method on pain reduction measured by the numerical scale of pain (NRS). The probands' research group was initially subjected to an initial examination and subsequently instructed to a regular home exercise. During the course of the investigation, probands were asked for changes in pain intensity each month for a 3month test period, and these values were subsequently evaluated. The result of this study was to determine whether this method is suitable for the treatment of impingement syndrome and whether its application has a positive effect on reduction of pain compared to baseline values measured NRS.
