Methods for determination of specific proteins and their effective use in clinical diagnosis of monoclonal gammopathy

Abstract:

Disertační práce se zabývá metodami stanovení bílkovin, pomocí kterých je možno detekovat monoklonální protein a tím vyhledávat nemocné s monoklonálními gamapatiemi. Historicky nejdéle se využívá vyšetření elektroforézy bílkovin v séru. Sensitivita záchytu monoklonální komponenty je 500 – 2000 mg/l, v závislosti na elektroforetické pohyblivosti a přítomnosti bílkovin v alfa nebo beta frakci. Elektroforéza bílkovin v moči je citlivější, vzhledem k nízké hodnotě celkové bílkoviny se vyšetřují koncentrované vzorky a dosahuje se v ideálním případě citlivosti 10-40mg/l. V praxi této senzitivity nelze dosáhnout u vzorků moče s vysokým obsahem celkové bílkoviny a se spektrem bílkovin odpovídajícím sérovému rozdělení, ani u vzorků, u kterých dochází k polymerizaci volných lehkých řetězců a ty jsou na elektroforéze rozděleny do několika linií. V 80. letech minulého století byla zavedena do rutinní diagnostiky elektroforéza s následnou imunofixací, jejíž citlivost pro záchyt monoklonální komponenty je asi 10x vyšší (120-250mg) ve srovnání s elektroforézou. V centrech, která se věnují nemocným s monoklonální gamapatií, nahradila ve screeningu elektroforézu bílkovin. Nejnovějším testem, který je nyní k dispozici pro rutinní diagnostiku, je stanovení volných lehkých řetězců imunoglobulinů v séru. Jedná se o turbidimetrické nebo nefelometrické měření, ve kterém se používají polyklonální protilátky zaměřené proti vnitřnímu epitopu volných lehkých řetězců, který je skryt v kompletní molekule. Pokud je u nemocného produkována pouze forma kompletní molekuly monoklonálního imunoglobulinu, tímto testem zachycena není. Metoda má zatím své limitace, které souvisí s měřením monoklonálních, strukturálně stejných molekul a závisí tedy na vlastnosti protilátky, jak dobře rozpozná monoklonální formu nebo její polymerní formy. Při měření se musí respektovat doporučený postup ředění, protože není dosahováno lineárních výsledků při různém ředění. Dosud také není k dispozici mezinárodní standardní materiál. Přes tyto nedostatky je klinická využitelnost stanovení volných lehkých řetězců významná. Jejich měření se stalo součástí mezinárodních i českých postupů pro screening pacientů s monoklonálními gamapatiemi. Cílem práce bylo na souboru nemocných, u kterých byla diagnostikována monoklonální gamapatie, zjistit diagnostickou sensitivitu doporučovaných vyšetření, zvláště se zaměřením na vyšetřování volných lehkých řetězců v séru. Pacienti byli vyšetřeni v ambulanci Interní hematoonkologické kliniky, bylo provedeno vstupní biochemické vyšetření, které kromě rutinních analýz zahrnovalo provedení elektroforézy v séru a zahuštěné moči, imunofixaci v séru (SIFE) a zahuštěné moči (UIFE) a stanovení volných lehkých řetězců v séru (FLC). Podle diagnózy byli pacienti rozděleni na dvě skupiny, skupinu s diagnózou maligní monoklonální gamapatie a skupinu s dg. monoklonální gamapatie neurčeného významu (MGUS). U obou skupin byla zhodnocena diagnostická senzitivita jednotlivých metod a jejich kombinací. V souboru 281 pacientů s dg. maligní monoklonální gamapatie bylo označeno nejvíce pacientů jako pozitivních (98,6%) při vyšetření maximální varianty vyšetření (SIFE+UIFE+FLC). Nebyli zachyceni celkem čtyři pacienti, tři pacienti s dg. mnohočetný myelom nesekretorický a jeden pacient s dg. ložiskový plazmocytom. Ve skupině s dg. MGUS to bylo 99,3% pacientů. Současná doporučení Mezinárodní pracovní skupiny pro mnohočetný myelom (IMWG) z roku 2009 doporučují jako standardní laboratorní vyšetření pro diagnostiku provádět elektroforézu a imunofixaci v séru společně se stanovením volných lehkých řetězců v séru. to označených pacientů v souboru maligní monoklonální gamapatie (97,9%) se nelišil (p 0,523) od maximální varianty vyšetření. Nebylo označeno šest pacientů, čtyři pacienti s dg. mnohočetný myelom nesekretorický, jeden pacient s dg. ložiskový plazmocytom a jeden pacient s dg. mnohočetný myelom. Ve skupině MGUS to bylo 98,6% pacientů (p0,562). Dle našich výsledků počet označených pacientů vyšetřením imunofixace v séru i v moči byl 96,1%, ale též se statisticky významně nelišil od maximální varianty (p 0,07). Nebylo zachyceno celkem jedenáct pacientů, devět s diagnózou mnohočetný myelom nesekretorický a dva pacienti s dg. ložiskový plazmocytom. Významný podíl pacientů ve skupině s maligní monoklonální gamapatií představují pacienti se sníženou funkcí ledvin (n 123). Byli hodnoceni jako samostatná skupina. Nejvyšší diagnostickou sensitivitu mělo u této skupiny pacientů použití trojkombinace metod (SIFE+UIFE+FLC) a kombinace metod vyšetřených v séru (SIFE+FLC) – bylo zachyceno 99,2% pacientů. Při hodnocení poměru FLC byly použity standardní i tzv. renální referenční meze. Použitím renálního referenčního poměru FLC se počet označených pacientů na hranici významnosti 5% snížil (p 0,06), ale počet pacientů označených v kombinaci s ostatními vyšetřeními zůstal stejný (99,2%). Použití renálního referenčního poměru FLC při hodnocení v kombinaci s imunofixací v séru nesnižuje diagnostickou sensitivitu vyšetření. Skupiny pacientů se sníženou a normální funkcí ledvin byly navzájem porovnány. Ve skupině pacientů se sníženou funkcí ledvin bylo označeno více pacientů provedením všech jednotlivých vyšetření i jejich kombinací. Statisticky významný rozdíl byl nalezen pouze při hodnocení jednotlivých vyšetření. Ze skupiny pacientů maligní monoklonální gamapatie je zvlášť hodnocena skupina pacientů s mnohočetným myelomem Bence Jonesova typu, mnohočetným myelomem nesekretorickým a primární AL amyloidózou. Stanovení FLC znamená nejvyšší diagnostický přínos pro skupinu pacientů s nesekretorickým nebo oligosekretorickým myelomem. Pro pacienty s dg. maligní monoklonální gamapatie představuje hodnocení FLC společně s imunofixací v séru (eliminace vyšetření moče) vyšetření nejvyšší diagnostické sensitivity.

Abstract:

The thesis deals with the methods of protein determination by means of which it is possible to detect a monoclonal protein and therefore to search for patients with monoclonal gammopathy. The test which has historically been used for the longest time makes use of the electrophoresis of serum proteins. The sensitivity of detection of monoclonal components is 500 - 2000 mg / l, depending on the electrophoretic mobility and on the presence of proteins in the alpha or beta fraction. Electrophoresis of proteins in urine is more sensitive as, given the low amount of total protein, concentrated samples are examined and the sensitivity obtained in the ideal case is 10-450mg / l. In practice, this sensitivity can not be achieved in urine samples with high content of total protein and with the protein spectrum corresponding to serum distribution of proteins, not even in samples in which free light chains are polymerized and they are thus divided into several lines by electrophoresis. In the 1980's, electrophoresis with subsequent immunofixation has been introduced into routine diagnostics; its sensitivity of monoclonal components detection is about 10 times higher (120-250mg) compared to electrophoresis. In centers devoted to patients with monoclonal gammopathy, it has replaced electrophoresis in protein screening. The newest test which is now available for routine diagnostics is the determination of free light chains of immunoglobulins in the serum. This is a nephelometric or turbidimetric measurement, in which polyclonal antibodies directed against an internal epitope of the free light chains which are hidden in a complete molecule are used. If the patient produces only a complete form of the monoclonal immunoglobulin molecule, this test does not detect it. The method has its limitations related to the measurement of monoclonal, structurally identical molecules and thus depends on the properties of the antibody and its ability to correctly recognize the monoclonal or polymeric form. When measuring, the recommended procedure of dilution must be respected as linear results are not achieved in various dilutions. An international standard material is not available yet. Despite these shortcomings, the clinical applicability of free light chains determination is significant. Their measurement became a part of international as well as of Czech screening procedures for patients with monoclonal gammopathy. The aim of this work was to determine the diagnostic sensitivity of the recommended tests focusing particularly on the investigation of free light chains on a set of patients who were diagnosed with monoclonal gammopathy. The patients were examined at the hemato-oncology unit, the initial biochemical tests which apart from routine analysis included also the electrophoresis of serum and concentrated urine, serum (SIFE) and concentrated urine (UIFE) immunofixation and determination of free light chains in serum (FLC) were carried out. According to their diagnosis, the patients were divided into two groups, a group with malignant monoclonal gammopathy and a group with gammopathy of undetermined significance (MGUS). The diagnostic sensitivity of each method and their combinations was evaluated in both groups. In the group of 281 patients with the diagnosis of malignant monoclonal gammopathy the largest number of patients was identified as positive (98.6%) when using the maximal combination of test examination variants (SIFE+UIFE+FLC). A total of four patients was missed, 3 patients with the diagnosis of myeloma oligosectretory or nonsecretory and one patient with the diagnosis of extramedullary plasmacytoma. er of positively identified patients in the group of malignant monoclonal gammopathy (97.9%) did not differ (p 0.523) from the those identified using maximal variations of the examination. Six patients were missed, four patients with the diagnosis myeloma oligosectretory or nonsecretory, one patient with a diagnosis of extramedullary plasmacytoma and one patient with the diagnosis of multiple myeloma. In the group of MGUS, the number was 98.6% of patients (p0,562). According to our results, the number of patients identified by testing immunofixation in serum and urine was 96.1%, but it also did not differ statistically significantly from the maximal variations (p 0.07). Eleven patients were missed, nine of them with the diagnosis of myeloma oligosectretory or nonsecretory and two patients with extramedullary plasmacytoma. A significant proportion of patients in the group of malignant monoclonal gammopathy is represented by patients with impaired renal function (n 123). This group was evaluated separately. The highest diagnostic sensitivity in this group of patients was using triple therapy methods (SIFE+UIFE+FLC) and the combination of the methods tested in serum (SIFE+FLC) - 99.2% of patients were rendered. When evaluating the FLC ratio, I used the standard as well as the renal reference range. By using the ratio of the renal reference, the number of patients identified at the border of significance of 5% decreased (p 0.06), but the number of patients identified in combination with other tests remained the same (99.2%). Using the renal reference FLC ratio for evaluation in combination with immunofixation in the serum does not reduce the diagnostic sensitivity of the tests. The group of patients with impaired renal function was compared with a group of patients with a normal renal function (n158) whose glomerular filtration rate (EGF) had been calculated according to MDRD> 1.0 ml / s. In the group of patients with impaired renal function, more patients were identified by performing all the various tests and their combinations. A statistically significant difference was not found when evaluating the combinations of tests. A significant difference was achieved when evaluating individual examinations. With the use of immunofixation of serum and urine and with the measurement of FLC more patients were captured in the group of patients with impaired renal function. From the group of patients with malignant monoclonal gammopathy, the group of patients with multiple myeloma Bence Jones type, multiple myeloma non secretory and primary AL amyloidosis was evaluated separately. Determining the FLC has the highest diagnostic yield in the group of patients with oligosecretory myeloma.
