Newborn screening for cystic fibrosis, molecular diagnosis, statistical evaluation

Abstract:

Nowadays a newborn screening (NS) is more or less established in all developed countries in the world. However, neither the number nor even the type of screened diseases is uniform. This is due to both historical development, but particularly the economic situation of individual countries. The aim of NS is to find the disease in the preclinical stage, which enable us to intervene before irreversible damage of the organism. Cystic fibrosis (CF) is the most common autosomal recessive disease and satisfies all criteria that are necessary for its inclusion in NS program. That´s why the newborn screening in Czech Republic was expanded also by this disease and CR became one of nine European Union countries, which have CF screening established in the nationwide NS. The aim of my bachelor thesis is a summary of knowledge about NS as in general as specifically in the form of monitor NS CF at the level of molecular diagnosis. In my thesis I target the detection of mutation in the CFTR gene, which is associated with this disease, using the method of multiplex ARMS. In the last part, there are evaluated NS CF results in FN Brno obtained in the period 1.12.2009 – 28.2.2011.

Abstract:

Novorodenecký screening (NS) je v súčasnej dobe vo väčšej či menšej miere zavedený vo všetkých vyspelých štátoch sveta. Avšak počet ani druh screenovaných ochorení nie je jednotný. Je to dôsledkom jednak historického vývoja, ale najmä ekonomickej situácie jednotlivých krajín. Cieľom NS je vyhľadávanie ochorenia v predklinickom štádiu, čo umožňuje zasiahnuť pred nenávratným poškodením organizmu. Cystická fibróza (CF) predstavuje najčastejšie sa vyskytujúce autozomálne recesívne ochorenie a spĺňa všetky kritériá potrebné pre jej zaradenie do programu NS. Práve preto sa NS v Českej republike na jeseň roku 2009 rozšíril aj o toto ochorenie a ČR sa tak stala jednou z 9 krajín Európskej únie, ktoré majú zavedený screening CF v rámci celoplošného NS. Cieľom mojej bakalárskej práce je súhrn poznatkov o NS ako obecne, tak aj konkrétne, a to v podobe sledovania NS CF na úrovni molekulárne genetickej diagnostiky. V práci sa zameriavam na detekciu mutácií v géne CFTR, ktorý je asociovaný s týmto ochorením, metódou multiplex ARMS. V závere budú vyhodnotené výsledky NS CF vo FN Brno získané v období 1.12.2009 – 28.2.2011.
