Physiotherapy for a client with postpartum brachial plexus paresis

Abstract:

Cíl práce: Cílem této bakalářské práce je přispět k řešení fyzioterapie u pacientky po poporodní paréze brachiálního plexu. Metodika: Hlavní část práce je koncipována ve formě kazuistiky pacientky s diagnózou poporodní parézy brachiálního plexu, která je řešena konzervativně. Pacientka absolvovala 8 terapeutických jednotek v průběhu od 4. 10. 2023 do 18. 1. 2024. Všechny terapie byly realizovány po konzultaci s fyzioterapeuty z oblastní nemocnice v Mladé Boleslavi. Po souhlasu pacientky s vypracováním kazuistiky byl zpracován vstupní kineziologický rozbor. Součástí vstupního vyšetření je komplexní kineziologický rozbor, který zahrnuje antropometrii, goniometrii, specifické neurologické vyšetření, testy HSSP, svalový test dle prof. Jandy, vyšetření jemné motoriky a anamnézu pacientky. Na základě vstupního kineziologického vyšetření je stanovena krátkodobá fyzioterapeutická intervence v délce trvání 16 týdnů s celkovým počtem 8 terapií a v době trvání terapeutické jednotky 60 - 90 minut. Počáteční terapie byly zaměřené zejména na uvolnění hypertonických svalů, uvolnění měkkých tkání a zvýšení rozsahu pohybu v ramenním kloubu pomocí metody PNF. K posílení HSSP jsme využili metodu DNS dle Koláře, dále jsme využili práci s Thera - Bandem, overballem či pomůcky pro nácvik jemné motoriky a další. Pacientka byla vždy edukována o provedení autoterapie. V průběhu rehabilitačního programu bylo po 4 terapiích provedeno kontrolní vyšetření. Po uplynutí skončení fyzioterapeutické intervence byl vypracován výstupní kineziologický rozbor, na základě jehož byl stanoven cíl a dlouhodobý plán fyzioterapie. Výsledky: Na základě krátkodobého plánu fyzioterapie se podařilo zlepšit rozsah pohybu v ramenním kloubu do flexe o 25° a extenze o 10°, abdukce o 40°, vnější rotace o 10°, vnitřní rotace o 5°, horizontální addukce o 10° a horizontální abdukce o 5°. Dále došlo ke zvýšení svalové síly při pohybu do extenze, abdukce, vnitřní rotace v ramenním kloubu ze stupně 3 na stupeň 4. Stejně tak došlo ke zlepšení svalové síly v loketním kloubu do flexe a extenze ze stupně 3 na stupeň 4. Jako poslední došlo ke zlepšení úchopů se subterminální opozicí palce a ukazováku a úchop s laterální opozicí. Závěr: Konzervativní terapie ve formě fyzioterapie byla částečně úspěšná pro zlepšení stavu pacientky - zvýšený RP ramenního kloubu, zvýšení svalové síly pletence ramenního.

Abstract:

Objective : The aim of this bachelor thesis is to contribute to the physiotherapy management of a client with postpartum brachial plexus paresis. Methodology. The patient underwent a total of 8 therapy units from October 4, 2023 to January 18, 2024. All therapies were performed in consultation with physiotherapists from the regional hospital in Mladá Boleslav. After the patient consented to the case report, an initial kinesiological analysis was performed. The initial examination includes a comprehensive kinesiological analysis, which includes anthropometry, goniometry, specific neurological examination, HSSP tests, muscle test according to Prof. Janda, fine motor examination and patient's history. On the basis of the initial kinesiological examination, a short-term physiotherapy intervention of 16 weeks with a total of 8 sessions and a duration of 60-90 minutes per therapeutic unit is determined. Initial therapies were mainly focused on relaxation of hypertonic muscles, soft tissue relaxation and increasing the range of motion in the shoulder joint using the PNF method. To strengthen the HSSP we used the DNS method according to Kolar, as well as work with Thera - Band, overball or fine motor training aids and others. The patient was always educated about the implementation of self therapy. In the course of the rehabilitation program, a follow-up examination was performed after 4 therapies. At the end of the physiotherapy intervention, a final kinesiological analysis was performed to determine the goal and long-term plan of physiotherapy. Results: the short-term physiotherapy plan resulted in a 10° improvement in shoulder joint range of motion into flexion 15° and extension 10° abduction 40° external rotation, 5° internal rotation, 10° horizontal adduction and 5° horizontal abduction. In addition, muscle strength in extension, abduction and internal rotation at the shoulder joint improved from grade 3 to grade 4. Similarly, muscle strength at the elbow joint in flexion and extension improved from grade 3 to grade 4. Finally, grip with subterminal thumb and index finger opposition and grip with lateral opposition improved. Conclusion: Conservative therapy in the form of physiotherapy was partially successful in improving the patient's condition - increased RP of the shoulder joint, increased muscle strength of the brachial plexus.
