Hereditary trombophilia - Factor V Leiden

Abstract:

Hluboká žilní trombóza je závažný zdravotní stav/choroba ohrožující život nemocného. Tato bakalářská práce se zabývá problematikou Leidenské mutace, která zvyšuje riziko vzniku žilní trombózy u svého nositele. Je řazena mezi uznávané hereditární trombofilie. Leidenská mutace je mutace v genu pro plazmatický koagulační faktor V, která je způsobena záměnou báze adeninu za guanin v pozici 506 a je dominantně autozomálně dědičná. Tato bodová mutace vede k záměně aminokyseliny argininu za glutamin, což způsobuje rezistenci na aktivovaný protein C. Důsledkem toho je pomalejší štěpení aktivovaných faktorů V a VIII a tím pádem zvýšená generace trombinu, který může v cévním řečišti významně podporovat vznik trombu. Trombóza je vždy multifaktoriální geneze, tudíž samostatná trombofilní mutace nezpůsobí rozvoj žilní trombózy, ale v kombinaci s dalšími rizikovými faktory se možnost vzniku trombózy mnohanásobně zvětšuje. Hlavními cíli bakalářské práce jsou zpracování rešeršní studie na téma Leidenská mutace faktoru V z odborné literatury a vyhodnocení průzkumu prováděného ve Fakultní nemocnici Brno na oddělení klinické hematologie. Cílem průzkumu bylo zjistit, do jaké míry tato bodová mutace ovlivňuje dotazované pacienty ve smyslu jejich životního stylu. U žen se zaměřuje na důležitý získaný rizikový faktor trombózy, hormonální antikoncepci, který taktéž s rozvojem žilní trombózy může úzce souviset.

Abstract:

Deep vein thrombosis is a serious medical condition/disease which threatening life of patient. This bachelor´s thesis concerns with the issue of Factor V Leiden which increases risk of the vein thrombosis origin at its holder. It is arranged to validated hereditary thrombophilia. Factor V Leiden is mutated form in gene for plasmatic coagulant factor V which is caused by substitution of adenine base into guanin at position 506 and it is dominantly hereditary. This mutation changes amino acid from arginine to glutamine which causes resistance to activated protein C. As a consequence of this there is a slower fission of activated factors V and VIII and so increased trombin generation which can significantly support a clot formation. Thrombosis is always multi factor genesis, therefore thrombophilia mutation itself does not cause the vein thrombosis development however by combination with other risk factors the possibility of vein thrombosis is much increased. The main goal of this bachelor´s thesis is processing the Factor V Leiden research study from technical literature and evaluation of the research performed by the Faculty Hospital Brno at Department of Clinical Hematology. The goal of the research was to find out how much this mutation affects respondent patients within the meaning of their lifestyle. In women the research focuses on the important risk thrombosis factor - hormonal contraception which can be related to the vein thrombosis development as well.
