Cognitive reserve as a valuable construct in monitoring individuals with multiple sclerosis

Abstract:

U 50 až 60 % lidí s roztroušenou sklerózou dochází ke kognitivnímu deficitu, který může výrazně ovlivňovat kvalitu života jedince. Kognitivní rezerva patří k již známým protektivním faktorům před působením neurodegenerativních procesů a to jak u roztroušené sklerózy, tak i například u Alzheimerovy demence a Parkinsonovy choroby. V práci jsme sledovali jak se kognitivní funkce u 667 pacientů (Ž = 460, M = 207) v průběhu 2 až 4 let měnily, a jestli míru poklesu kognice, měřené pomocí baterie BICAMS, ovlivňuje pacientova premorbidní kognitivní výkonnost. Dosaženou výkonnost jsme měřili pomocí Českého testu čtení slov a porovnávali jsme, jestli slouží jako lepší ukazatel rezervy než pouhé vzdělání. Samotné vzdělání predikovalo pouze počáteční úroveň kognice, ale její pokles v čase už nedokázal vysvětlit. Oproti tomu test čtení slov souvisel jak s výchozí hodnotou testu, tak i s mírou poklesu. Navíc určil skupinu (NART >30 bodů), u které nedochází ke statisticky významnému poklesu kognitivních funkcí. To bychom potenciálně mohli nazvat jako protektivní faktor, či využít jako prognostický údaj v praxi. Test čtení slov představuje poměrně jednoduchou a nenáročnou metodu s dobrým informační přínosem a jeho zavedení do běžné praxe může být užitečné.

Abstract:

Between 50 and 60 % of people with multiple sclerosis experience cognitive deficits that can significantly affect an individual's quality of life. Cognitive reserve is one of the already known protective factors against the effects of neurodegenerative processes in multiple sclerosis, as well as in Alzheimer's dementia and Parkinson's disease, for example. In this study, we investigated how cognitive function in 667 patients (F = 460, M = 207) changed over a 2- to 4-year period and whether the degree of cognitive decline, as measured by the BICAMS battery, is influenced by the patient's premorbid cognitive performance. We measured cognitive reserve using the Czech Word Reading Test and compared whether it serves as a better indicator of reserve than education alone. Education alone only predicted the initial cognitive level but could not explain its decline over time. In contrast, the word-reading test was related to both the baseline test score and the rate of decline. In addition, it identified a group (NART >30 points) that did not experience statistically significant cognitive decline. This could potentially be termed a protective factor or used as a prognostic indicator in practice. The word reading test is a relatively simple and uncomplicated method with good informational benefit, and its introduction into routine practice may be useful.
