Subjective experiencing of patients in the early post-transplant phase in the process of bone marrow transplantation

Abstract:

Tato práce se zabývá subjektivním prožíváním hematoonkologických pacientů v časném potransplantačním období v procesu transplantace kostní dřeně. Nejprve je věnována pozornost obecným i aktuálním poznatkům z oboru hematoonkologie a konkrétně transplantace hematopoetických kmenových buněk. V empirické části je zaměřena pozornost na samotný cíl práce, tedy na zjištění a zhodnocení subjektivního prožívání pacientů hospitalizovaných na jednotce intenzivní péče Hematoonkologické kliniky ve Fakultní Nemocnici Ostrava. Sběr dat pro účely práce byl realizován prostřednictvím polostrukturovaných rozhovorů s pacienty. Výzkum sledoval faktory ovlivňující biologickou, emocionální, sociální a kognitivní oblast prožívání pacientů léčených metodou transplantace hematopoetických buněk. Výzkumem se podařilo identifikovat nejčastější fyzické obtíže pacientů, či zjistit míru snášenlivosti nastalých problémů při záměrné podpůrné profylaxi. V oblasti emocionálního prožívání vzhledem k nutnosti izolace na transplantační jednotce, bylo zjištěno, že největší podporu nalézají pacienti ve vlastní rodině, ale neméně též v ošetřujícím personálu a jeho přístupu. Výzkum také odhalil, že míra informovanosti a adaptace na proces léčby měla zásadní význam pro to, jaký postoj k léčbě a ke svému zdraví pacienti zaujali. Zajímavé bylo zjištění, že většina pacientů si byla plně vědoma faktu, že léčba není schopna zajistit stoprocentní výsledek a onemocnění se tak může v budoucnu vrátit. V závěrečné části jsou výsledky interpretovány a porovnávány s výstupy jiných autorů, kteří se zabývali obdobným výzkumem.

Abstract:

This bachelor thesis deals with subjective experiencing of haematoonkologic patients in the early post-transplant phase in the process of bone marrow transplantation. First it focuses on general knowledge and recent findings in the area of haematooncology and specifically transplantation of haematopoetic stem cells. Empirical part focuses on the aim of the thesis, i.e. finding and assessing of subjective experiencing of patients hospitalized at the Intensive Care Unit of the Departement of Haematooncology of the University Hospital Ostrava. Collection of data for the purposes of this bachelor thesis was realized by semi-structured interviews with patients. The research observed factors influencing biological, emotional, social and cognitive area of experiencing of patients treated with the method of transplantation of haematopoetic cells. The research has identified the most common physical complications of patients or has discovered the degree of tolerance of complications during deliberate supportive profylaxis. In the field of emotional perception considering the need of isolation on the transplantation unit, it has been found that patients find the greatest support in their own family, but no less in the staff members and their approach. Research has also revealed that the level of awareness and adaptation to the treatment process has been crucial to the patients' attitude to their treatment and health. Interestingly, it was found that most patients were fully aware of the fact that the treatment was not able to provide a 100% result and the disease could return in the future. In the final part the results are interpreted and compared to findings discovered by other authors, who conducted similar research in the past.
