Physiotherapy after Aortic Coarctation Surgery in Child Patient

Abstract:

Úvod: Tématem mé bakalářské práce je fyzioterapie po operaci koarktace aorty u dětského pacienta. Hlavním důvodem výběru tohoto tématu byla jeho neobvyklost a dobrá datová srovnatelnost už v průběhu terapie. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u pacientů po prodělané kardiochirurgické operaci volbou vhodných fyzioterapeutických metod. Metodika: Spolupráce s pacientem vznikla necelých 8 měsíců po jeho operaci a trvala po dobu 15 týdnů. Jako první byl proveden vstupní kineziologický rozbor, který se skládal z antropometrie, goniometrie, svalového testu dle Jandy, vyšetření trofiky a kůže, hodnocení hemodynamických parametrů a speciálních testů. Společně s rozborem bylo provedeno i zátěžové EKG vyšetření. Na základě výsledků vstupního vyšetření byl stanoven krátkodobý fyzioterapeutický plán, který primárně spočíval v navýšení pacientovi kondice pomocí jízdy na rotopedu 2x - 3x týdně po dobu 15 týdnů. Mezi další prvky plánu patřilo odstranění patologií v oblasti levého ramenního kloubu, za užití konceptů BP Jarmily Čápové, PNF, Brügger koncept? a PFS, a také napravení dysbalance v oblasti pánve a břicha pomocí konceptů DNS, PNF. Po 8 týdnech bylo provedeno kontrolní vyšetření. Na konci 15. týdne proběhl výstupní kineziologický rozbor stejnou formou jako probíhal vstupní kineziologický rozbor. Z výsledků byl následně sestaven dlouhodobý fyzioterapeutický plán, který spočíval zejména v setrvání pacienta v pohybové aktivitě. Výsledky: Při výstupním kineziologickém vyšetření byly zaznamenány změny, především v klidové tepové frekvenci, kdy došlo k poklesu z původní hodnoty 79 tepů/min (ze dne 11.10.2019) na 61 tepů/min. Dále došlo k navýšení rozsahu pohybu levého ramenního kloubu do plné 180° flexe z původní 175° flexe. Také do abdukce se zvýšil rozsah pohybu z původních 170° na 175°. Narůst v rozsahu byla také naměřena v zevní rotace, kdy z původních 80° bylo po terapii dosaženo 85°, a vnitřní rotace, kdy z původních 70° bylo po terapii dosaženo 75°. Závěr: Fyzioterapeutická intervence byla pozitivní, neboť se podařilo splnit stanovené cíle a úkoly práce.

Abstract:

Introduction: The topic of my bachelor thesis is physiotherapy after aortic coarctation surgery in a child patient. The main reason for choosing this topic was its unusual nature. Objective: The aim of this work is to contribute to the solution of physiotherapy in patients after cardiac surgery by choosing suitable physiotherapeutic methods. Methods: The cooperation with patient has begun less than 8 months after his surgery and lasted for 15 weeks. At first the kinesiological analysis was performed on. It consisted of anthropometry, goniometry, Janda muscle test, aspect examination, trophic and skin examination, testing hemodynamic parameters, and special tests together with the ergometric ECG examination. Based on the results, the short-term physiotherapy plan was established. The main aim of this plan was to increase the patient's fitness by cycling on exercise bike twice or three times a week for 15 weeks. Other elements of the plan included the removal pathologies of the left shoulder joint using BP Jarmily Čápové, PNF, Brügger koncept? and PFS. It included also correcting pelvic and abdominal dysbalance using DNS, PNF concepts. After 8 weeks the check-up examination was performed. At the end of the 15th week, the output kinesiological analysis was performed in the same form as the input analysis. The long-term physiotherapeutic plan was compiled based on the results. It mainly consisted of physical activity. Results: The changes occurred during the output kinesiological examination, especially the change in the resting heart rate, when there was a decrease from the original value of 79 bpm (from 11.10.2019) to 61 bpm. Furthermore, the range of movement of the left shoulder joint increased to full 180° flexion from the original 175 ° flexion. Also, the range of motion to abduction increased from the original 170° to 175°. The increase in range was also measured in external rotation, when from the original 80° reached 85°, and internal rotation, when from the original 70° reached 75°. Conclusion: The physiotherapeutic intervention was positive because the goals and tasks of the work were accomplished.
