Assessment of the Hearing at Workers in Risk and Outside of Risk of Noise

Abstract:

Úvod: Sluch je jedním z nejdůležitějších lidských smyslů. Ukazatelem ztráty sluchu je sluchový práh, který můžeme charakterizovat jako nejnižší intenzitu zvuku, kterou je jedinec schopen slyšet. Cílem práce bylo porovnat průměrné sluchové prahy: mezi muži a ženami, mužů a žen s ISO normou 7029 a osob exponovaných a neexponovaných hluku v pracovním prostředí. Materiál a metody: Data byla získána z grantové studie IGA MZ ČR NT12246-5/2011 ? Epidemiologická a genetická studie frekvence sluchových vad. Součástí sběru dat bylo vyplnění dotazníku a vyšetření obou uší. Celkem se zmíněné studie zúčastnilo 27 722 osob, avšak pro účely porovnání s ISO normou byla v konečném zpracování použita data pouze od 13 498 osob a pro účely porovnání sluchových prahů mezi muži a ženami exponovaných a neexponovaných hluku v pracovním prostředí byla použita data od celkem 14 474 osob. Výsledky: Z výsledků vyplývá, že ženy téměř ve všech věkových skupinách a souborech mají na frekvencích do 2000 Hz statisticky významně vyšší ztráty sluchu oproti mužům. Od této frekvence byl sledován opačný trend. Při srovnání s ISO normou bylo zjištěno, že ženy mají téměř na všech frekvencích statisticky významně nižší ztráty sluchu, než předpokládá ISO norma. U mužů je to velmi obdobné, pouze na frekvencích 250 Hz a 500 Hz nebyly zjištěny statisticky významné rozdíly oproti normě. Ztráty sluchu u žen se v závislosti na pracovní expozici významně neliší. U mužů byly zjištěny statistické významné rozdíly mezi exponovanými a neexponovanými hluku v pracovním prostředí již od 18-ti let. Závěr: Ve všech věkových skupinách této studie měly audiogramy stoupající tendenci v závislosti na průměrných sluchových ztrátách a frekvenci. Tento trend v audiometrické křivce je typický pro věkem podmíněnou ztrátu sluchu. Zjištěné nižší hodnoty sluchových prahů ve srovnání s ISO normou mohou naznačovat, že populace v 21. století již není srovnatelná s populací, ze které vychází platná norma, která čerpá data ze studií z let 1959 až 1970. V pracovním prostředí s rizikem hluku byly zjištěny statisticky významné rozdíly mezi muži a ženami s větší četností než u osob bez rizika hluku. Maximální rozdíly ve středních hodnotách však nepřevyšovaly hodnotu 10 dB, z čehož může vyplývat, že hluk v pracovním prostředí se na ztrátách sluchu podílí jen malou měrou.

Abstract:

Introduction: Hearing is one of the most important human senses. Indicator of hearing loss is hearing threshold which can be characterized as the lowest level of sound that the individual is able to hear. The aim of the study was to compare the average hearing thresholds: between men and women; men and women with ISO 7029 norm and individuals exposed and unexposed to noise in the workplace. Materials and Methods: Data were obtained from an Epidemiological and Genetic Study of the Frequency of Hearing Loss (IGA MZ CR NT12246-5 / 2011). Part of the data collection was completed using a questionnaire and examining of both ears. In total 27,722 people participated in the study. In the final process, data from 13,498 and 14 474 persons were used for the purposes of comparison with ISO standard and for the purpose of comparing hearing thresholds between males and females exposed and unexposed to noise in the workplace, retrospectively. Results: The results showed that women have in almost all age groups at frequencies up to 2000 Hz significantly higher hearing loss than men. From frequency 3000 Hz was observed the opposite trend. When compared with the ISO standard, it was found that women had at almost all frequencies significantly less hearing loss than expected by the ISO standard. For men, the results were similar, only at frequencies of 250 Hz and 500 Hz there were no statistically significant differences compared to the norm. Differences in hearing loss in women, according to the occupational exposure were not signifiant. In males exposed and unexposed to noise in the workplace was observed hearing loss as early as at the age of 18 years. Conclusion: In this study audiograms had increasing tendency according to the average hearing loss and frequency in all age groups. This trend in audiometric curve is typical for age-related hearing loss. Identified lower auditory thresholds compared with the ISO standard may indicate that the population of the 21st century is no longer comparable with the population from 1959 to the 1970?s which the standard is based on. At work place with the risk of noise there were statistically significant differences between men and women at higher frequency than in those without the risk of noise. However, maximum differences in mean levels did not exceed 10 dB. This could mean that the noise in the work place contributes to hearing loss only to a small extent.
