Nursing Process of the Patient with Dementia

Abstract:

Tématem bakalářské práce je demence, jedna z nevýznamnějších a nejčastějších diagnóz v gerontopsychiatrii, a specifika ošetřovatelské péče u pacientů s tímto onemocněním. Hlavním cílem je popsat a analyzovat případ 76leté pacientky, která je hospitalizována na psychiatrickém oddělení pro ženy starší 65 let. Případová studie je jednou z metod kvalitativního přístupu. Zde je soustředěna na jednu pacientku s diagnózou F03 – Neurčená demence. V bakalářské práci je detailně popsáno období poskytování ošetřovatelské péče metodou ošetřovatelského procesu (10.1. 2020-12.2. 2020). K posouzení zdravotního stavu pacientky a utřídění informací ke stanovení ošetřovatelských problémů je využit koncepční model Dorothey Elizabethy Oremové, který je zaměřený na pacientčinu schopnost sebepéče. Ošetřovatelské diagnózy jsou stanoveny dle NANDA I taxonomie II. 2015-2017. Plány ošetřovatelské péče, jejich realizace a zhodnocení péče jsou podrobně popsány u ošetřovatelské diagnózy zaměřené na problém Akutní zmatenost 00128 a na rizikové ošetřovatelské diagnózy Riziko sníženého objemu tekutin (riziko dehydratace) 00028. Akutní zmatenost má dopad na kvalitu života pacientky. Efektivním plánovaním ošetřovatelských intervencí a jejich realizací se podařilo udržet pacientčinu orientaci místem a časem na dobré úrovni, eliminovat riziko dehydratace, dále zkvalitnit spánek, snížit deficit sebepéče, podpořit pacientčinu snahu zlepšit péči o své zdraví a předejít riziku pádu. Ošetřovatelská péče, která je velmi důležitou součástí léčby demence, musí být pacientům poskytována kvalitně s nezbytným respektem k jejich lidské důstojnosti. Výstupem bakalářské práce jsou doporučení pro praxi, jak pro rodinné příslušníky a nebližší okolí nemocného, tak pro všeobecné sestry.

Abstract:

The topic of this bachelor thesis is dementia, which is one of the most important and most common diagnoses in gerontopsychiatry, and specifics of care taking of patients with this disease. The main goal is to describe and analyze a case of a 76 years old woman, which is hospitalized in a psychiatric ward for women of ages 65 and more. Case study is one of the methods of qualitative research. In this paper, I focused on one female patient with the diagnosis F03 – A Nonspecific Dementia. This bachelor thesis describes in detail the time period of nursing via the method of care taking process (10.1. 2020-12.2. 2020). To determine the health and sort information properly to describe the correct issues that require care, I used The Self-Care Nursing Deficit Concepts by Dorothea Elizabeth Orem. The Nursing Diagnoses are set in compliance with NANDA standards, specifically by Taxonomy I and II (2015-1017). The care taking plans, their realization and evaluation are described in detail in the nursing diagnosis Acute Confusion 00128 and The Risk of Low Fluid Intake (Risk of Dehydration) 00028. Acute Confusion affects the patient‘s quality of life. The effective planning and execution of the nursing interventions achieved that the patient remained oriented in place and time on a satisfying level, eliminated the risk of dehydration, improved quality of sleep, decreased the self-care deficiency, supported the patient‘s efforts to improve her indenpendancy and reduced the risk of falling down. The nursing care, which is a very important part of the process of treating dementia, must be provided properly, with necessary respect to the basic human dignity. The output of this bachelor thesis are recommendations for practice for family members, people in close circle and nurses.
