Adherence in post-transplant care in children

Abstract:

V práci se zaměřujeme na adherenci (ve starším pojetí complianci) dospívajících v dětském transplantačním programu Fakultní nemocnice Motol. Cílem výzkumu je zjistit, jaké motivy dospívající s adherentním a nonadherentním chováním nejčastěji spojují s adherencí, resp. nonadherencí a následně pilotně ověřit námi vytvořený dotazník kognitivní orientace. Vycházíme při tom z teorie kognitivní orientace. Výzkum probíhal ve dvou fázích. V první fázi jsme do výzkumného souboru zařadili dospívající před a po transplantaci ledviny a jater, které jsme rozdělili do dvou skupin na základě jejich adherence a následně s nimi vedli polostrukturované rozhovory zaměřené na identifikaci motivů. Ke zjištěným motivům jsme přiřadili osobní i obecná přesvědčení a z těchto přesvědčení jsme vytvořili dotazník kognitivní orientace. Ve druhé částí výzkumu jsme tento dotazník administrovali druhé skupině dospívajících s adherentním a nonadherentním chováním a ověřovali jeho schopnost diskriminovat mezi adherentními a nonadherentními dospívajícími.

Abstract:

Adherence is essential for the success of the treatment in the transplant program. In the study we assess adherence (compliance) in adolescents in the pediatric transplant program of Motol University Hospital. The aim of the study is to identify motives associated with adherence/nonadherence by adherent and nonadherent adolescents and to verify a questionnaire of cognitive orientation created according to the theory of cognitive orientation. The study had two phases. In the first phase adolescents before and after the transplantation (liver and kidney) were included. Subsequently, two groups-adherent and nonadherent adolescents were created and interviewed to identify the motives for adherence and nonadherence. After that, we joined personal and general beliefs to motives and created a cognitive orientation questionnaire. In the second phase, we administered the questionnaire to adolescents with adherent or nonadherent behavior and verified the potential of the questionnaire to discriminate between adherent and nonadherent adolescents.
