Molecular epidemiology of macrolides-lincosamides-streptogramin B cross-resistance and mechanisms influencing its occurrence and transmission

Abstract:

Makrolidy, linkosamidy a streptogramin B (MLSB) jsou chemicky odlišné ale funkčně podobné skupiny antibiotik. Jejich účinek na bakteriální buňku je založen na vazbě k velké podjednotce ribozomu a blokaci syntézy proteinů. Patří mezi druhou nejčastěji používanou skupinu antibiotik pro léčbu pacientů v ambulantní péči v Evropě a makrolidy jsou zároveň čtvrtým nejprodávanějším antibiotikem ve veterinární medicíně. Ztráta účinku všech tří antibiotik této skupiny je spojena s jedním ze 40 erm genů, jejichž produkty modifikují specifické cíle v molekule rRNA. Při hodnocení významu pro veřejné zdraví a rizik spojených s detekcí těchto determinant rezistence je nutné také zohlednit jejich vazbu s dalšími geny, zejména pro rezistenci k tetracyklinu. Cílem této práce bylo zjistit, do jaké míry závisí šíření genu erm(B) ve střevě kuřat na různém selekčním tlaku antibiotik, jaká je úloha střevní mikroflóry jako zdroje determinant rezistence a jaký je genetický podklad odolnosti k makrolidům a tetracyklinu u enterokoků z hospodářských zvířat. Jak specifický, tak nespecifický selekční tlak antibiotik měl za následek šíření genu erm(B) ve střevě kuřat během antibiotické léčby i po jejím ukončení. Slepé střevo kuřat se zdá být více zatíženo erm(B) determinantami rezistence a nárůst abundance genu erm(B) během posledního dne odběru vzorků trusu v negativní kontrolní skupině je nejspíše důsledkem vylučování obsahu slepého střeva . Nejčastěji prokázanými geny rezistence ve slepém střevě kuřat byly determinanty rezistence k tetracyklinu a některé z nich byly lokalizovány na mobilních genetických elementech, které dle literatury nesou současně geny erm a tet. Společná lokace výše uvedených genů může vysvětlovat šíření erm(B) determinant během nespecifického selekčního tlaku antibiotik. Mezi enterokoky z hospodářských zvířat rezistentními k makrolidům a tetracyklinu byly identifikovány dva potenciálně nové plazmidy o délce 42 a 57 kb nesoucí jak erm, tak dvě determinanty rezistence tet, které pocházely ze dvou odlišných izolátů.

Abstract:

Macrolide, lincosamide and streptogramin B (MLSB) antibiotics are three chemically distinct, but functionally similar antibiotic classes. They all act by binding to the large ribosome subunit and blocking the protein synthesis. They are the second most commonly used antibiotic group in outpatient treatment in Europe and macrolides are the fourth most sold antibiotic in veteri nary medicine. Through specific target modification of the rRNA via the action of the products of one of the 40 erm genes, all three antibiotic classes are rendered ineffective. Therefore the genetic linkage to other resistance determinants, especially tetracycline is one of the factors that need to be accounted for when assessing the public health relevance and risk associated with the detection of these resistance determinants. The aim of the thesis was to explore in which extent spread of the erm(B) gene in the chicken gut depends on different antimicrobial selective pressures, what is the role of the intestinal microflora as a source of resistance determinants and what is the genetic background underlying the macrolide and tetracycline resistance in enterococci from livestock. Both specific and unspecific antibiotic selective pressures have led to the spread of the erm(B) gene in the chicken intestine during and after antibiotic treatment. The chicken caecum seems to be more burdened by erm(B) resistance determinants and caecal droppings are most likely the result of the relative increase in abundance of the erm(B) resistance gene on the last day of faeces sampling in the negative control group. The most common resistance genes identified in the chicken caecum were tetracycline resistance determinants, some of which were located on to mobile genetic elements that were described in literature of carrying both erm and tet genes. This co-localisation of the above mentioned genes could explain the spread of erm(B) determinants under non-specific antibiotic selective pressure. Among macrolide and tetracycline resistant enterococci from livestock, two possible new 42 and 57 kb long plasmids carrying both erm and two tet resistance determinants could be identified from two distinct isolates.
