The use of contrast-enhanced ultrasonography in classification of renal cystic lesions

Abstract:

Úvod. Naší snahou bylo zhodnotit možnosti kontrastní ultrasonografie (CEUS) při klasifikaci cystických ložisek ledvin a srovnat výsledky s výpočetní tomografií (CT). Metody. Do souboru byli zařazeni pacienti, kteří měli v období 2007-2013 provedeno vyšetření ledvin na CEUS a CT s nálezem ložiska, které mělo alespoň na jedné z metod cystický charakter. Ložiska byla hodnocena podle Bosniakovy klasifikace a byla buď ověřena histologicky nebo sledována alespoň 2 roky. Soubor tvoří 112 ložisek u 94 pacientů. Statisticky byla hodnocena korelace mezi metodami při Bosniakově klasifikaci, citlivost metod k detekci jednotlivých znaků používaných v Bosniakově klasifikaci. Hodnocena byla dále senzitivita a specificita metod při odlišení maligního a benigního ložiska jednak z pohledu Bosniakovy klasifikace a jednak z pohledu jednotlivých znaků. Data byla zpracována ROC analýzou s odhadem senzitivity, specificity a plochy pod křivkou a identifikací optimálního hraničního bodu. Pro identifikaci hraničního bodu byla použita metoda maximalizace součtu senzitivity a specificity. Odhad hraničního bodu byl následně použit pro výpočet poměru šancí. Pro hodnocení statistické významnosti byl použit Fisherův exaktní test. Výsledky. Celková shoda mezi oběma metodami byla 61 %, při rozdělení pouze na nechirurgické (kategorie I, II, IIF) a chirurgické (III, IV, solidní) léze byla shoda 90 %. Hranice mezi chirurgickými a nechirurgickými ložisky představovala i optimální hraniční bod, senzitivita CEUS pak byla výrazně vyšší než senzitivita CT (92,3 % vs. 69,2 %), specificita CEUS byla mírně nižší (90,9 % vs. 94,9 %). Při hodnocení obsahu bylo CT významně citlivější (p=0,006) pro detekci denzního obsahu než UZ pro detekci echogenního obsahu. Nález echogenního resp. denzního obsahu zvyšoval pravděpodobnost maligní povahy ložiska, na CEUS 30krát, na CT 7krát. CEUS byl statisticky významně citlivější než CT v detekci sept (53 % ložisek vs. 25 % ložisek, p<0,001) a také v detekci sycení sept (22 % vs. 10 % ložisek; p<0,001). V detekci sycení solidní složky nebyl prokázán statisticky významný rozdíl mezi oběma metodami (13,4 % ložisek na CEUS, 8,9 % na CT; p=0,228), na CEUS však představoval tento znak významnější faktor pro detekci malignity (zvýšení pravděpodobnosti malignity 384krát na CEUS, 78krát na CT; p<0,001). Závěr. Výsledky CEUS korelují s výsledky CT, CEUS tedy umožňuje klasifikovat cystická ložiska ledvin pomocí Bosniakovy klasifikace podobně jako CT. Senzitivita CEUS k detekci maligních ložisek je vyšší než u CT.

Abstract:

Introduction. Our aim was to evaluate the classification of cystic renal lesions with CEUS and compare the results with CT. Methods. The set included patients who were made in the 2007-2013 period CEUS and CT examination of renal lesion that had at least on one of the methods cystic character. Lesions were evaluated by Bosniak classification and either histologically verified or followed-up for at least two years. The set consists of 112 lesions in 94 patients. We made statistical evaluation of the correlation between the methods in Bosniak classification, sensitivity and specificity of both methods for the detection of features used in Bosniak classification. We further assessed the sensitivity and specificity of both methods for differentiating malignant and benign lesions from the viewpoint of Bosniak classification and that of individual features. Data were processed by ROC analysis with an estimated sensitivity, specificity, and area under the curve and identification of the optimal cut-off. To identify a cut-off maximum sum of sensitivity and specificity was used. Estimated cut-off was subsequently used to calculate the odds ratio. To assess statistical significance we used Fisher's exact test. Results. Overall agreement between both methods was 61 %, while with the division to non-surgical (Category I, II, IIF) and surgical (III, IV, solid) lesions agreement was 90 %. The boundary between surgical and non-surgical lesions represented an optimal cut-off point, then CEUS sensitivity was markedly higher than sensitivity of CT (92.3 % vs. 69.2 %), specificity of CEUS was slightly lower (90.9 % vs. 94.9 % ). When evaluating the content CT was significantly more sensitive (p=0.006) for detecting hyperattenuating content than ultrasound for detection of echogennic content. Echogenic/hyperattenuating content increased likelihood of malignant nature of the lesion, 30 times on CEUS, 7 times the CT. CEUS was significantly more sensitive than CT in detecting septa (53 % of lesions vs. 25 % of lesions, p<0.001) and in the detection of enhancement of septa (22 % vs. 10 % of lesions, p<0.001). In the detection of enhancement of solid component there was not a statistically significant difference between both methods (13.4 % of lesions on CEUS, 8.9 % on CT, p=0.228), but on CEUS this feature was more important factor for the detection of malignancy (increased likelihood of malignancy 384 times on CEUS, 78 times on CT, p<0.001). Conclusion. Results of CEUS correlate with CT, CEUS is therefore able to classify cystic renal lesions with Bosniak classification like CT. The sensitivity for detection of malignant lesions is higher on CEUS than on CT.
