Physiotherapy for intervertebral disc herniation

Abstract:

Úvod: Tato bakalářská práce se zabývá problematikou výhřezu meziobratlové ploténky v oblasti bederní páteře. V teoretické části jsou zpracované teoretické aspekty o herniaci disku. Praktická část se věnuje kazuistice pacientky s výhřezem, kde se využívají získané znalosti a aplikují se fyzioterapeutické metody, které povedou k naplnění cílů a úkolů práce. Cíl práce: Přispět k řešení problematiky fyzioterapie u pacienta s výhřezem meziobratlové ploténky pomocí fyzioterapeutických metod a konceptů. Metodika: Praktická část mé práce obsahuje zpracování kazuistiky 47leté ženy s výhřezem meziobratlové ploténky v oblasti bederní páteře, která začíná odebráním anamnézy (rozhovorem) s následným provedeným vstupním kineziologickým rozborem. Na základě výsledků ze vstupního vyšetření jsem stanovila krátkodobý plán fyzioterapie. Poté následovalo po dobu šesti týdnů dvanáct terapií, s frekvencí dvě terapie za týden, kdy každá terapie trvala 60 minut. Během terapií jsem používala techniky měkkých tkání (TMT), PIR (postizometrická relaxace), PFS (postfacilitační strečink), MOB (mobilizace), trakce, ACT (akrální koaktivační terapie), DNS (dynamická neuromuskulární stabilizace), SMS (metoda senzomotorické stimulace), metodu Ludmily Mojžíšové a Brügger koncept. Po skončení fyzioterapeutických intervencí jsem provedla výstupní kineziologický rozbor a navrhla dlouhodobý plán fyzioterapie. Výsledky: Podle pacientova subjektivního vnímání došlo k úlevě od bolesti a obtíží spojených s onemocněním. Z výsledků vyplývá, že se pacientce zlepšilo držení těla, navodila se aktivita stabilizace páteře a zlepšil se dechový stereotyp. Prokázalo se zvýšení rozsahu pohybu kyčelních kloubů do flexe, extenze, VR (vnitřní rotace) a ZR (zevní rotace). Zvýšila se svalová síla břišních svalů, vnitřních rotátorů kyčelních kloubů, extenzorů a adduktorů kyčelních kloubů. Obnovila se kloubní vůle SI (sakroiliakálního) skloubení. Snížilo se zkrácení svalů dolních končetin (DKK) a paravertebrálních svalů, a také se snížil hypertonus některých svalů. Podařilo se obnovit posunlivost a protažitelnost měkkých tkání v oblasti zad. Závěr: Terapii vnímám z větší části za úspěšnou. Dozvěděla jsem se spoustu teoretických znalostí v problematice výhřezu ploténky a praktických poznatků, které se dají využít ve fyzioterapeutických intervencích.

Abstract:

Introduction: This bachelor thesis deals with the problem of intervertebral disc prolapse in the lumbar spine. In the theoretical part the theoretical aspects about disc herniation are elaborated. The practical part is devoted to a case study of a patient with a prolapse, where the acquired knowledge is used and physiotherapeutic methods are applied, which will lead to the fulfilment of the aims and objectives of the thesis. The objective: To contribute to solving the problem of physiotherapy in patients with intervertebral disc prolapse using physiotherapy methods and concepts. Methodology: The practical part of my thesis includes a case report of a 47-year-old woman with intervertebral disc prolapse in the lumbar spine, which begins with taking a medical history (interview) followed by an initial kinesiological analysis. Based on the results from the initial examination, I determined a short-term physiotherapy plan. This was followed by twelve sessions over a six-week period, with a frequency of two sessions per week, with each session lasting 60 minutes. During the therapies, I used soft tissue techniques (TMT), PIR (postisometric relaxation), PFS (post-facilitation stitching), MOB (mobilization), traction, ACT (acral coactivation therapy), DNS (dynamic neuromuscular stabilization), SMS (sensorimotor stimulation method), the Ludmila Mojžíšová method and the Brügger concept. After the physiotherapy interventions, I performed an exit kinesiological analysis and proposed a long-term physiotherapy plan. Results: According to the patient's subjective perception, there was relief from the pain and difficulties associated with the disease. The results show that the patient's posture improved, spinal stabilization activity was induced and breathing stereotype improved. There was an increase in the range of motion of the hip joints into flexion, extension of VR (external rotation) and ZR (external rotation). Muscular strength of the abdominal muscles, hip internal rotators, hip extensors and hip adductors increased. SI (sacroiliac joint) joint clearance was restored. Shortening of the hip and paravertebral muscles was reduced, and muscle hypertonus was also reduced. It was possible to restore the mobility and stretchability of the soft tissues in the back area. Conclusions: I see the therapy as largely successful. I have learned a lot of theoretical knowledge on the issue of disc prolapse and practical knowledge that can be used in physiotherapy interventions.
