Abstract:

Nedoslýchavost se stává závažným celosvětovým problémem. Sluchová vada je 2. nejzávažnější vada po postižení mentálním. Nedoslýchavost má v jakémkoli věku obrovský dopad na mezilidskou komunikaci, psychosociální pohodu, kvalitu života a ekonomickou nezávislost. Dle WHO na celém světě zhruba 466 milionů lidí trpí nedoslýchavostí a z toho je 34 milionů dětí mladších 15 let. Ostatní nedoslýchaví jsou dospělí a očekává se, že do roku 2050 vzroste počet nedoslýchavých na 900 milionů. Celkové počty se nadále zvyšují, protože více států vyšetřuje sluch a tato data jsou dostupná pro světové statistiky. Prevence a včasný záchyt sluchové vady znamená dřívější poskytnutí adekvátní péče, tím pádem menší hendikep a lepší kvalitu života. Cílem práce je formou rešerše zmapovat situaci vyšetřování sluchu ve světě, navrhnout kroky pro implementaci screeningového vyšetření sluchu pro jednotlivé věkové kategorie v rámci národního programu pro ČR. V bakalářské práci bylo zjištěno, že novorozenecký screening sluchu je velmi dobře popsán. I když je na celém světě 34 miliónů dětí s poruchou sluchu, tak pouze dvě třetiny států provádějí univerzální novorozenecký screening. Provádění screeningu sluchu u předškolních dětí není tak rozšířené, jako u novorozenců. V Americe se screening sluchu provádí ve 4 letech a v Evropě v 5ti letech. Bohužel v Africe a Asii ještě nejsou zavedeny programy screeningu sluchu předškolních dětí. Další screening sluchu by měl následovat u dětí školního věku, který ale není tak běžný jako novorozenecký screening sluchu. V Americe se screening sluchu provádí v 6 a 8 letech. V Africe se provádí pouze vyšetření sluchu u rizikových dětí, v Asii po nástupu do školy a v Evropě na preventivních prohlídkách lékařem pro děti a dorost. Přestože se incidence nedoslýchavosti zvyšuje s věkem, tak problematika screeningu sluchu dospělých není dostatečně zkoumána a popisována. Vyšetření sluchu jsou v Americe a Evropě prováděna na preventivních prohlídkách praktickým lékařem. Problematika screeningu sluchu seniorů začíná být více diskutovaným tématem a začíná se o provádění screeningu sluchu zabývat více specialistů. Kdy pouze v Americe se sluch seniorů vyšetřuje na preventivních prohlídkách, a to pouze formou dotazníků a sporadicky se tak děje i v Evropě. V České republice je doposud novorozenecký screening sluchu plošně prováděn pouze ve třech krajích, a to v Moravskoslezském kraji, Královehradeckém kraji a Pardubickém kraji. Roku 2018 vydalo Ministerstvo zdravotnictví ČR Metodický pokyn k provádění screeningu sluchu u dětí ve věku 5 let. Další screening sluchu u dětí školního věku, v dospělosti a ani u seniorů se v České republice neprovádí. Jelikož je screening sluchu v České republice dobře podchycen u novorozenců a mladších dětí, měli bychom se teď zaměřit na dospělou a starší populaci. S technickým pokrokem a širokým využíváním mobilních telefonů, by se jako metoda screeningu sluchu dospělých a starších lidí mohla začít využívat mobilní aplikace pro kontrolu sluchu, která nebudou pro lékaře časově náročné a jejich provádění není drahé. Také se mohou začít využívat pro screening sluchu dospělých Dotazník problémů se sluchem pro dospělé, a pro seniory Dotazník problémů se sluchem pro seniory, které se využívají v mnoha zemích. Jedná se o dobře ověřený nástroj screeningu sluchu a jeho výhodou je, že je levný. Na léčbu nedoslýchavosti stát v současné době vynakládá velké množství finančních prostředků, které by se zavedením screeningových programů měli snížit, a tak ulevit zdravotnímu rozpočtu. Abstract:

Hearing loss is becoming a serious global problem. Hearing impairment is the second most serious defect after mental handicap. According to the WHO, around 466 million people worldwide suffer from hearing loss. Of these, 34 million were under 15 years, 7.5 million under 5 years. The other hard of hearing were adults, and by 2050 number hard of hearing are expected to rise to 900 million. Prevention and early detection of hearing impairment means earlier provision of adequate care, thus less disability and better quality of life. The aim of the thesis is to map the situation of hearing examination in the world in the form of a research, to propose steps for the implementation of screening examination of hearing for individual age categories within the national program for the Czech Republic. In the bachelor's thesis, it was found that neonatal hearing screening is very well described. Although there are 34 million children with hearing impairment worldwide, only two-thirds of countries conduct universal neonatal screening. Hearing screening in preschool children is not as widespread as in newborns. In America, hearing screening is performed at the age of 4 and in Europe at the age of 5. Unfortunately, preschool hearing screening programs are not yet in place in Africa and Asia. Further hearing screening should follow in school-age children, but this is not as common as neonatal hearing screening. In America, hearing screening is performed at 6 and 8 years of age. In Africa, only hearing tests are carried out on at-risk children, in Asia after school and in Europe on preventive medical examinations for children and adolescents. Although the incidence of hearing loss increases with age, the issue of adult hearing screening is not sufficiently studied and described. Hearing tests are performed in America and Europe on preventive examinations by a general practitioner. The issue of hearing screening in seniors is becoming a more discussed topic and more specialists are beginning to deal with the implementation of hearing screening. When only in America is the hearing of seniors examined at preventive examinations, and only in the form of questionnaires, and this happens sporadically in Europe as well. In the Czech Republic, neonatal hearing screening has so far been widely performed in only three regions, namely in the Moravian-Silesian Region, the Hradec Králové Region and the Pardubice Region. In 2018, the Ministry of Health of the Czech Republic issued a Methodical Instruction for Hearing Screening in Children Aged 5. Further hearing screening in school-age children, adults and seniors is not performed in the Czech Republic. As hearing screening in the Czech Republic is well captured in newborns and younger children, we should now focus on the adult and older population. With technical progress and the widespread use of mobile phones, mobile hearing control applications could be used as a method of screening for the hearing of adults and the elderly, which will not be time-consuming for physicians and are not expensive to implement. They can also be used to screen the Adult Hearing Problems Questionnaire for adults, and the Senior Hearing Problems Questionnaire for seniors, which are used in many countries. It is a well-proven hearing screening tool and its advantage is that it is cheap. The state currently spends a large amount of money on the treatment of hearing loss, which should be reduced with the introduction of screening programs, thus relieving the health budget. It must also be expected that the input costs for the implementation of hearing screening programs will be higher, but with the prevention and early diagnosis of these programs, these costs will return in the future.
