Physiotherapy in painful shoulder syndrome

Abstract:

Úvod: Tato bakalářská práce je na téma fyzioterapie u syndromu bolestivého ramene. Poznáním nových informací jsem se snažila souhrnně zpracovat problematiku syndromu bolestivého ramene. Cíl práce: Cílem bakalářské práce je zpracovat a přispět k řešení problematiky fyzioterapie u pacientky se syndromem bolestivého ramene, přesněji u zmrzlého ramene. Metodika: Hlavní částí mé bakalářské práce je kazuistika pacientky se syndromem bolestivého ramene (zmrzlého ramene). S pacientkou jsem spolupracovala 6 týdnů, během kterých proběhlo 10 terapií, každá byla v délce 30 min. Na začátku rehabilitace jsem udělala vstupní kineziologický rozbor (aspekce, palpace, antropometrie, goniometrie, vyšetření pohybových stereotypů, speciální testy na ramenní kloub - vyšetření joint play, testy instability, testy impingement sy a testy rotátorové manžety, neurologické vyšetření, vyšetření zkrácených struktur a hypermobility), z jehož hodnot jsem sestavila krátkodobý fyzioterapeutický plán. Během rehabilitační léčby jsem udělala jedno kontrolní vyšetření, které bylo přibližně v polovině terapií. Během cvičební jednotky bylo využito následných fyzioterapeutických konceptů a metod (TMT, PIR, PNF, trakce, kineziotape, DNS, Brügger koncept, Bazální posturální programy dle J. Čápové atd) k ošetření měkkých tkání, zvýšení rozsahu pohybu a svalové síly v ramenním kloubu. Před ukončením rehabilitační léčby jsem udělala výstupní kineziologický rozbor, kdy jsem na základě výsledků stanovila dlouhodobý fyzioterapeutický plán. Výsledky: Zlepšení jsem viděla v posunlivosti fascií a měkkých tkání v oblasti ramenního kloubu a krční páteře. Největší změna se projevila v hybnosti ramenního kloubu, v první řadě do FL, ABD, ZR a VR v ramenním kloubu (FL z 70° na 160°, ABD 60° na 160°, ZR 35° na 60° a VR 30° na 60°). Závěr: Na závěr mé práce jsem zhodnotila výsledky vstupního a výstupního kineziologické rozboru. Rehabilitační léčba kladně ovlivnila život pacientky, a to tím že se výrazně zvětšil rozsah pohybu a významně zmenšila bolest v ramenním kloubu.

Abstract:

Introduction: This bachelor thesis is on topic of physiotherapy in painful shoulder syndrome. By learning new information, I strived to process the issue of painful shoulder syndrome in summary. The objective: The aim of the bachelor's thesis is to process and contribute to solving the problem of physiotherapy in patient with painful shoulder syndrome, more precisely in the frozen shoulder. Methodology: The main part of my bachelor's thesis is the case study of a patient with painful shoulder syndrome - a frozen shoulder. I have been working with the patient for 6 weeks, during which 10 half-hour therapies took place. At the beginning of the rehabilitaion treatment, I carried out an initial kinesiological analysis (assortment, palpation, anthropometry, goniometry, examination of movement stereotypes, special tests on the shoulder joint - joint play examination, tests of instability, tests of impingement sy and tests of rotator cuff, neurological examination, examination of shortened structures and hypermobility), from whose values I compiled a short-term physiotherapeutic plan. During rehabilitation treatment, I did one check-up, which was about halfway through therapy. During the exercise units, physiotherapy concepts and methods (TMT, PIR, PNF, traction, kinesiotape, DNS, Brügger concept, Basal postural programs to J. Čápová etc.) were used in order to treat soft tissues, increase the range of movement and muscle strength in the shoulder joint. Before completing rehabilitation treatment, I did an exit kinesiological analysis, where I established a long-term physiotherapy plan based on the results. Results: I saw improvements in the shifting of fasciy and soft tissues in the shoulder joint and cervical spine. The biggest change was reflected in the mobility of the shoulder joint, primarily to FL, ABD, ZR and VR in the shoulder joint (FL from 70° to 160°, ABD 60° to 160°, ZR 35° to 60° and VR 30° to 60°). Conclusions: At the end of my work, I evaluated the results of the input and output kinesiological analysis. Rehabilitation treatment positively affected the patient's life by significantly increasing the range of movement and significantly reducing pain in the shoulder joint.
