Nursing Process for Patient with Extracorporeal Membrane Oxygenation

Abstract:

Tématem bakalářské práce je ošetřovatelský proces u pacienta s extrakorporální membránovou oxygenací (ECMO). ECMO je metoda používaná u nemocných v kritickém stavu k překlenutí nejtěžší fáze selhání krevního oběhu nebo selhání plic. Hlavním cílem práce je analýza základních postupů komplexní ošetřovatelské péče u pacienta s kardiogenním šokem či oběhovou zástavou na podkladě infarktu myokardu, léčeného s pomocí systému ECMO. Bakalářská práce je rozdělena na teoretickou a praktickou část. Teoretická část se zabývá problematikou infarktu myokardu, kardiogenním šokem, srdeční zástavou a kardiopulmonální resuscitací. Závěr teoretické části je zaměřený na popis principů použití extrakorporální membránové oxygenace u různých stavů selhání srdce či plic. V praktické části je na základě získaných informací zpracována kazuistika pacienta s akutním infarktem myokardu, komplikovaným srdeční zástavou. Byla zahájena kardiopulmonální resuscitace, která vedla k obnovení krevního oběhu, následně však došlo k rozvoji kardiogenního šoku. Po příjezdu do kardiocentra byl pacient napojen na extrakorporální membránovou oxygenaci. Praktická část je zpracována podle funkčních modelů zdraví Marjory Gordonové. U konkrétního pacienta jsou stanoveny ošetřovatelské diagnózy, které jsou následně rozpracovány. Závěrem praktické části je vypracování doporučení pro praxi. Akutní infarkt myokardu je závažný, život ohrožující stav. I v případě, že je infarkt komplikován selháním krevního oběhu, s pomocí systému ECMO lze tuto kritickou fázi překonat. Jedná se však o ošetřovatelsky velmi náročný proces.

Abstract:

The thopic of this thessis is the nursing process in patient with extracorporeal membrane oxygenation (ECMO). ECMO is a rescue method used in critically ill patients that can stabilize patients with circulatory collapse or respiratory failure. The objective of the thessis is the analysis of basic procedures in the complex nursing care in patients with cardiogenic shock or cardiac arrest after myocardial infarction treated with ECMO. It is divided into the two parts: theoretical and practical. The theoretical part is focused on the myocardial infarction, cardiogenic shock, cadiac arrest and cardiopulmonary resuscitation. Furthermore, the theoretical part describes the basic principles of ECMO therapy in cases with severe cardiac or respirátory failure. The practical part depicts the patient with acute myocardial infarction complicated by cardiac arrest. Cardiopulmonary resuscitation was successfull and spontaneous blood circulation was rapidly restored; however, subsequently severe cardiogenic shock developed. The patient was transferred to the cardiovascular center and treated with ECMO. This practical part is based on the nursing assessment theory known as Gordon's functional health patterns. The nursing diagnoses are determined for this patient and then individually elaborated. Finally the practical part includes recommendations for clinical practice. Acute myocardial infarction is a serious, life-threatening condition. Myocardial infarction can be complicated with circulatory failure and the insertion of ECMO could bridge this critical phase to recovery or other therapeutic procedures. However, the ECMO therapy is associated with very complex nursing process. t heme of this bachelor work is nursing care of adult patients on extracorporeal membrane oxygenation. The main focus of this paper is nursing management of a critically ill patient with a wide range of complex needs. The patient discussed in this work suffered cardiac arrest as a complication of myocardial infarction, developing cardiogenic shock during his admission into medical centre. The choice of treatment was extracorporeal membrane oxygenation. The bachelor thesis is divided in to theoretical and practical part. The first chapter of the theoretical part of this paper describes causes of myocardial infarction, cardiogenic shock and principles of cardiopulmonary resuscitation. The last chapter is focused on extracorporeal membrane oxygenation (ECMO), its different types and *medical uses. {*impact on the human body.} The practical part of this bachelor work contains one study case demonstrating the utilization of ECMO in patients with myocardial infarction, complicated by cardiac arrest and evolving into cardiogenic shock. Upon arrival at the cardiocenter, the patient is connected to extracorporeal membrane oxygenation. This part uses a nursing assessment theory known as Gordon's functional health patterns. Nursing diagnoses are determined for a particular patient, which are then elaborated. The conclusion of the practical part is a recommendation for practice.
