Neuropsychologický výkon u pacientů v akutní fázi bipolární afektivní poruchy

Neuropsychological performance in patients with acute phase of bipolar disorder

Abstract:

Úvod. Kognitivní funkce jsou v současnosti důležitou součástí klinického popisu bipolární afektivní poruchy vzhledem k dopadu kognitivního deficitu na psychosociální fungování na kvalitu života pacientů. Nicméně přes vzrůstající zájem o toto téma za poslední roky, nejsou doposud dostupná data ohledně profilu a dynamiky kognitivních funkcí u bipolární poruchy konzistentní. Naším cílem je zhodnocení neuropsychologického profilu u pacientů v akutní fázi bipolární afektivní poruchy (BAP) v porovnání se zdravými kontrolami (ZK). Metody. Celkem 53 pacientů s BAP, z toho 16 v hypománii, 17 v mánii, 20 v depresi a 23 zdravých kontrol bylo vyšetřeno komplexní neuropsychologickou testovou baterií. Mezi neuropsychologické metody jsme zařadili testy zaměřené na exekutivní funkce, pozornost, paměť, psychomotorické tempo a verbální fluenci. Rovněž jsme se zabývali kognitivními výkony ve vztahu ke klinicky relevantním charakteristikám jako je věk nástupu onemocnění, délka onemocnění, počet hospitalizací, posuzovací objektivní škály MADRS a YMRS. Ke statistické analýze získaných dat jsme použili Kruskal-Wallisův test, Mann-Whitneyův U test a k zhodnocení míry vztahu mezi proměnnými byl použit Spearmanův koeficient pořadové korelace. Výsledky. Pacienti s bipolární poruchou vykazují kognitivní poškození v oblasti psychomotorického tempa, pozornosti, verbální paměti a v menší míře u exekutivních funkcí a vizuální paměti. BAP ve srovnání se ZK vykazují statisticky významně horší výkon ve všech kognitivních zkouškách kromě iniciačního času Londýnské věže TOL tit (U= 480, p = 0,142). Skupina ZK vykazuje výkony ve všech testech v rámci normy. Srovnání kognitivních výkonů ve fázi HM, M, D a ZK odráží statisticky významný rozdíl ve všech kognitivních testech kromě TOL tit (H= 6,464; p= 0,091), kde se 4 skupiny statisticky významně neliší. Kognitivní výkony v akutních fázích HM, M, a D nevykazují statisticky významný rozdíl ani v jedné z kognitivních zkoušek. Po zohlednění klinických charakteristik jsme zaznamenali nejtěsnější korelace mezi věkem nástupu onemocnění a verbální pamětí AVLT 1 (r = -0,452; p < 0,01), AVLT Sum (r = -0,383; p < 0,01), AVLT 30 (r = -0,370; p < 0,01). Závěr. V naší studii pacienti v akutní fázi BAP vykazují významně horší výkon ve všech kognitivních doménách oproti ZK. Kognitivní poškození jsme zaznamenali v doméně psychomotorického tempa, pozornosti, verbální paměti a v menší míře u exekutivních funkcí a vizuální paměti. Pacienti v HM, M a D fázi se v kognitivních výkonech významně nelišili. Z klinických charakteristik pouze věk nástupu onemocnění nejvíce koreloval s kognitivním výkonem.

Abstract:

Background. Cognitive functions are currently an important part of the clinical description of bipolar disorder due to the impact of cognitive deficits on psychosocial functioning on patients' quality of life. However, despite the increasing interest in this topic over the past few years, the available data on the profile and dynamics of cognitive function in bipolar disorder are not yet consistent. The aim of this paper is to evaluate the neuropsychological profile in patients in the acute phase of bipolar disorder (BD) compared to healthy controls (HC). Methods. A total of 53 patients with BD, of which 16 were in hypomania (HM), 17 were in mania (M), 20 were in depression (D) and 23 healthy controls (HC), were examined by a complex neuropsychological assessment tools. Among the neuropsychological methods, tests focused on executive functions, attention, memory, psychomotor speed and verbal fluency were included. Cognitive performance in relation to clinically relevant characteristics such as the onset of illness, duration of illness, hospitalization, MADRS and YMRS assessment objective scales were also looked at. For data analysis the Kruskal-Wallis test, the Mann-Whitney U test were chosen as suitable methods. Spearman´s Rank-Order Correlation was used to measure the relationship between the variables. Results. Patients with bipolar disorder exhibit cognitive impairment in the areas of psychomotor speed, attention, verbal memory and, to a lesser extent, in executive functions and visual memory. BD compared to HC showed statistically significantly worse performance in all neuropsychological tests except with the Tower of London (ToL) total initiation time (U = 480, p = 0.142). The HC group performed within standards with all tests. The comparison of cognitive performances in the HM, M, D phases and HC reflects a statistically significant difference in all cognitive tests except for ToL total initiation time (H = 6.464; p = 0.091) where the 4 groups are not statistically significant. Cognitive performance in acute phases of HM, M, and D did not show a statistically significant difference in either of the cognitive tests. After taking into account the clinical characteristics, the closest correlation between the onset of the illness and the verbal memory AVLT 1 (r = -0,452; p <0,01), AVLT Sum (r = -0,383; p <0,01), AVLT 30 (r = -0,370; p < 0,01) were recorded. Conclusions. In this study patients in the acute phase of BD exhibited significantly worse performance than HC in all cognitive domains. Cognitive impairment was recorded in the domain of psychomotor speed, attention, verbal memory and to a lesser extent executive functions and visual memory. Patients in the HM, M, and D phases did not differ significantly in cognitive performance. From the clinical characteristics, only the age of the patient at the onset of illness correlated the most with cognitive performance.
