Quality of life of patients with implantable cardioverter-defibrillator

Abstract:

Cíl: Hlavním cílem bakalářské práce bylo zjistit kvalitu života pacientů s ICD a porovnat výsledky s populačními normami. Dále posoudit informovanost pacientů, zkušenosti s výboji a výskyt komplikací. Metodika: Průzkumný soubor tvořilo 82 pacientů s ICD ve věku do 65 let, minimálně 6 měsíců po implantaci, kteří navštívili arytmologickou ambulanci nebo byli hospitalizováni na kardiologických odděleních. Šetření bylo realizováno v období od října po prosinec 2019. Ke sběru dat byl použit dotazník Světové zdravotnické organizace WHOQOL-BREF, doplněný o otázky vlastní konstrukce. Výsledky: Pacienti hodnotili kvalitu života nejčastěji jako dobrou (34 %) a se svým zdravím byli spokojeni průměrně (37 %). Po implantaci prodělalo výboj 30 pacientů (37 %), komplikace se vyskytly u 28 pacientů (34 %). Oproti běžné populaci dosahovali pacienti nižší kvality života ve všech oblastech kromě prostředí. Nejlépe hodnotily kvalitu života osoby žijící v partnerském vztahu, respondenti zaměstnaní, s vyšším stupněm vzdělání a pacienti 3 roky po implantaci. Zhoršené vnímání kvality života bylo zjištěno u žen, pacientů ve věku 40-49 let, u nezaměstnaných a pacientů časně po implantaci. Nižších hodnot dosáhli také pacienti, kteří prožili výboj nebo prodělali komplikaci. Pacienti nebyli spokojeni se svou energií, pracovním výkonem a negativními pocity, naopak dobře hodnotili podporu blízkých, smysl života a podmínky k životu. Závěr: Kvalita života pacientů s ICD byla snížena především v oblastech prožívání a fyzického zdraví. Je potřeba zaměřit se na podporu pacienta s cílem pomoci mu přijmout přístroj do života a minimalizovat u něj psychický stres.

Abstract:

Aim: The main aim of the bachelor thesis was to assess the quality of life of patients with an ICD and to compare the results with population norms. Furthermore, to determine the level of patients' awareness, experiences with device discharges and occurrence of complications. Methods: The researched group consisted of 82 patients with an ICD aged up to 65 years of age, at least 6 months after implantation, who visited an arrhythmological outpatient clinic or were hospitalized at cardiology departments. The research was conducted between October and December of 2019. For the purposes of collecting the data a WHOQOL-BREF questionnaire developed by the World Health Organization was used, supplemented by questions of my own construction. Results: The patients assessed the quality of their life most frequently as good (34 %) and they were satisfied with their health in an average manner (37 %). After implantation, 30 patients received an ICD shock (37 %) and complications occurred for 28 patients (34 %). In comparison to the healthy population, the patients achieved lower scores of quality of life in all domains except for the environment. The life quality was assessed most positively by patients who were living in a partnership, who were employed, those with a higher level of education and by patients 3 years after the implantation. Impaired perception of quality of life was found in women, patients between 40 and 49 years of age, unemployed respondents and patients right after the implantation. Patients who experienced shock or had a complication also achieved lower quality of life. The patients were not satisfied with their energy levels, work performance and the presence of negative feelings. On the contrary, they appreciated the support by their close ones, the meaning of life and life conditions. Conclusion: The quality of life of patients with an ICD was decreased particularly in the areas of physical and psychical health. It is recommended to focus on providing support to these patients, trying to help them to accept the device as a part of their life and to minimize their psychological stress.
