Abstract:

Cíl: Cílem bakalářské práce bylo přiblížit problematiku kojení a augmentace. Zjistit, zda augmentace ovlivňuje kojení a popřípadě zachytit nejčastější komplikace, které mohou kojení s prsními implantáty ovlivnit. Data byla zpracována metodikou praxe založené na důkazech, kdy použité studie byly ze zahraničních zdrojů. Metodika: Vyhledání vhodných studií bylo provedeno zadáním klíčových slov v českém a anglickém jazyce v elektronických databázích. Následně proběhlo u vhodných bibliografických odkazů a odborných časopisů manuální prohledávání dat. Do praktické části byly zahrnuty studie zabývající se tématikou kojení s prsními implantáty z období 1970 ? 2016. Po důkladné analýze dat bylo do praktické části práce zahrnuto 8 prospektivních studií, 2 randomizované kontrolované studie, 2 retrospektivní studie a 4 syntézy prospektivních studií. Výsledky: Po zhodnocení dostupných studií, bylo zjištěno, že ženy s prsními implantáty mají větší potíže s kojením oproti ženám, bez prsních implantátů. Přestože prsní implantáty nemají vliv na kvalitu mateřského mléka, byly zjištěny další faktory, které mohou laktaci ovlivnit. Závěr: Po zpracování výsledků, byly zjištěny faktory, které mohou negativně ovlivnit kojení. Je potřeba dostatečně informovat ženy během kojení i před samotnou plastickou operací. Na základě výsledků byly zpracovány informační materiály, které poslouží k větší informovanosti a edukaci pacientek a porodních asistentek. Abstract:

Aim: The aim of this diploma thesis was to deal with the topic of breastfeeding and breast augmentatiton and to find out if breast augmentatiton influences breastfeeding. The aim was also to record complications which may most often influence breastfeeding with breast implants. The methodics used to compile the data was evidence based practice where studies from foreign sources were used. Methodics: Searching suitable studies was done by typing key words in Czech and English language in electronic databases. Data in suitable bibliographic references and special magazines were searched manually. Studies which deal with the topic of breastfeeding with breast implants during the period 1970 - 2016 were included in practical part. After thorough data analysis eight prospective studies, two randomized controlled trails, two retrospective studies and four synthesis of prospective studies were included in practical part. Results: The evaluation of accessible studies showed that women with breast implants have more difficulties with breastfeeding compared to women without breast implants. Although breast implants do not have influence on the quality of mother's milk, other factors which may influence lactation were found. Conclusion: Factors which may influence breastfeeding in a negative way were found after compiling the results. It is necessary to inform women during their breastfeeding and even before the plastic operation. The results were used to compile informative materials which may help to inform and educate patients and midwives.
