Peripheral Neuropathy In Diabetics With Lower Extremity Arterial Disease

Abstract:

Úvod: Pacienty s diabetem doprovází řada zdravotních komplikací. Jednou z častých mikroangiopatických komplikací je diabetická neuropatie, jejíž nejčastější podoba periferní neuropatie (PN) postihuje nervy vedoucí zejména do dolních končetin. PN komplikuje zdravotní stav 50 % diabetikům. Jejím charakteristickým znakem je porucha citlivosti v oblasti chodidel. Tento fakt je nutno zohlednit, protože PN může zamaskovat příznaky ischemické choroby dolních končetin (ICHDKK), jakožto časté makroangiopatické komplikace diabetu. V tomto důsledku nemusí být ICHDKK v iniciálních fázích odhalena, čímž narůstá riziko kardiovaskulárních onemocnění. Zároveň jsou obě tyto zdravotní komplikace hlavními rizikovými faktory syndromu diabetické nohy. Cíl: Cílem práce bylo porovnat výsledky orientačního vyšetření PN s výsledky dotazníkového šetření u pacientů s diabetem 2. typu a tyto zjištěné výsledky porovnat s objektivně prokázanou přítomností ICHDKK. Metodika: Soubor 21 diabetiků nejprve vyplnil standardizovaný dotazník zjišťující přítomnost PN na základě subjektivních příznaků. Poté bylo provedeno orientační vyšetření periferní neuropatie (OVPN), které zahrnovalo vyšetření povrchového čití monofilamentem 10 g (PN MNF) a hlubokého (vibračního) čití ladičkou 128 Hz (PN LAD). Zároveň na stejném souboru bylo provedeno vyšetření na přítomnost ICHDKK, a to pomocí metody ankle brachial indexu (ABI), toe brachial indexu (TBI) a duplexní ultrasonografie DUS, která byla zvolena jako vyšetřovací standard. Následně bylo provedeno porovnání výsledků dotazníkového šetření s výsledky OVPN. Dále byly vypočteny parametry validity dotazníkového šetření. Na závěr bylo provedeno porovnání zjištěné přítomnosti PN s objektivně prokázanou ICHDKK. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Při porovnání výsledků OVPN s výsledky dotazníkového šetření nebyl zjištěn statisticky významný rozdíl (p = 0,18). Shoda mezi těmito metodami byla 76 %, avšak byla zjištěna jen při detekci zdravých probandů. Zatímco metodou OVPN bylo zachyceno 19 % pozitivních probandů, dotazníkovým šetřením pouze 5 %. Při hodnocení parametrů validity vykazovalo dotazníkové šetření v porovnání s OVPN, která byla v této práci zvolena jako referenční metoda, senzitivitu 0 %, specificitu 94 %, pozitivní prediktivní hodnotu (PPV) 0 % a negativní prediktivní hodnotu (NPV) 80 %. Při porovnání dílčích vyšetření OVPN, tedy PN MNF a PN LAD, s dotazníkovým šetřením nebyl taktéž zjištěn statisticky signifikantní rozdíl. Zjištěné výsledky přítomnosti PN se následně využily také pro porovnání s objektivně prokázanou přítomností ICHDKK dle DUS a také dle výsledků ABI a TBI. Při porovnání výsledků jednotlivých metod stanovení ICHDKK s výsledky metody OVPN nebyl zjištěn statisticky významný rozdíl. P hodnoty v rámci jednotlivých porovnání byly větší než hladina významnosti 5 %. Závěr: Výsledky dotazníkového šetření v porovnání s OVPN nevykazovaly rozdílné výsledky při detekci pacientů s PN. Při porovnání metod ke stanovení přítomnosti ICHDKK v porovnání s OVPN nebyl zjištěn statisticky významný rozdíl mezi přítomností PN a přítomností ICHDKK na vyšetřovaných končetinách.

Abstract:

Introduction: Patients with diabetes are accompanied by a number of health complications. One of the common microangiopathic complications is diabetic neuropathy, the most common form of which is peripheral neuropathy (PN), which affects the nerves leading most commonly to the lower limbs. PN complicates the health of 50% of diabetics. Its characteristic feature is a sensitivity dysfunction in the foot area. This fact must be considered, as PN can mask the symptoms of lower extremity arterial disease as a common macroangiopathic complication of diabetes. As a result, lower extremity arterial disease may not be detected in the initial stages, increasing the risk of cardiovascular disease. Simultaneously, both of these health complications are the main risk factors for diabetic foot syndrome. Aim: The aim of the work was to compare the results of the basic examination of PN with the results of a questionnaire conducted on patients with type 2 diabetes, and to compare these results with the objectively proven presence of lower extremity arterial disease. Methodology: First, a group of 21 diabetics completed a standardized questionnaire determining the presence of PN based on subjective symptoms. Basic examination of peripheral neuropathy (BEPN) was then performed, which included examination of the surface sensation with 10 g monofilament (PN MNF) and deep (vibrational) sensation with a 128 Hz tuner (PN LAD). At the same time, the same group was examined for the presence of ICHDKK, using the method of ankle brachial index (ABI), toe brachial index (TBI) and duplex ultrasonography DUS, which was chosen as the examination standard. Subsequently, the results of the questionnaire were compared with the results of the BEPN. Furthermore, the parameters of the validity of the questionnaire were calculated. Finally, a comparison of the detected presence of PN with an objectively proven lower extremity arterial disease was performed. Statistical tests were evaluated at a significance level of 5%. Results: When comparing the results of BEPN with the results of the questionnaire, no statistically significant difference was found (p = 0.18). A correlation between these methods was 76%, however found only in the detection of healthy probands. While 19% of positive probands were detected by the BEPN method, only 5% was detected by the questionnaire. Evaluation of the validity parameters of the questionnaire in comparison to BEPN, which was chosen as a reference method in this research, showed a sensitivity of 0%, specificity of 94%, a positive predictive value (PPV) of 0% and a negative predictive value (NPV) of 80%. When comparing the partial examinations of BEPN, i.e. PN MNF and PN LAD, with the questionnaire, no statistically significant difference was found. The obtained results of the presence of PN were subsequently used also for comparison with the objectively proven presence of lower extremity arterial disease according to DUS and also according to the results of ABI and TBI. When comparing the results of individual methods of ICHDKK assessment with the results of the BEPN method, no statistically significant difference was found. The P values within each comparison were greater than the 5% significance level. Conclusion: The results of the questionnaire in comparison with BEPN did not show different results in the detection of probands with PN. When comparing the methods for assessing the presence of lower extremity arterial disease in comparison with BEPN, no statistically significant difference was found between the presence of PN and the presence of lower extremity arterial disease on the examined limbs.
