Perception of Life Values by Patients in Pallitive Care

Abstract:

Percepcia životných hodnôt u pacientov v paliatívnej starostlivosti. Diplomová práca sa venuje mapovaniu hodnotovej orientácie u pacientov v paliatívnej starostlivosti, pacientov zomierajúcich. Sústredili sme sa na súčasnú hodnotovú orientáciu a snažili sme sa vypátrať, ako sa zmenila v porovnaní s hodnotami vyznávanými počas života pacienta. Brali sme ohľad na zviazanie bilancovania života s konštruktmi aktuálnej kvality života a vnímania zmyslu života, ktoré v práci teoreticky rozpracovávame. Opierame sa o teoretické východiská kruhového modelu systému hodnôt, o definície kvality života a prístupy existenciálnej psychológie a logoterapie a existenciálnej analýzy k smrti a fenoménu zmyslu života. V empirickej časti práce sme interpretatívnou fenomenologickou analýzou rozobrali štyri rozhovory prevedené s pacientmi v terminálnom štádiu onemocnenia. Otázky k semištrukturovanému rozhovoru sme konštruovali na základe teoretických východísk kruhového systému hodnôt podľa koncepcie Schwartza, dotazníkov hodnotiacich kvalitu života a testov zisťujúcich zmysluplnosť života. Diskutujeme podobné témy vystávajúce z výpovedí pacientov, a hľadáme možné zovšeobecnenie záverov pre psychologická prácu v oblasti paliatívnej starostlivosti. Uvádzame aj obmedzenia výskumu, ktoré takisto diskutujeme. V práci prispievame k priblíženiu subjektívneho pohľadu terminálne nemocného pacienta na jeho uplynulý život, a na proces jeho bilancovania z perspektívy hodnotových orientácii, kvality a zmyslu života. Silným nástrojom pozitívneho bilancovania je možnosť vnímania nemoci ako príležitosti a hľadanie zmyslu priamo v situácii onemocnenia.

Abstract:

Perception of Life Values by Patients in Palliative Care. This master thesis is focused on research of life values as percepted by patients in palliative care, dying patients. We have concentrated on the present value orientation and tried to discover how it has changed in comparison with values preferred during the lifetime of the patient. The bond between evaluation of ones lifetime with constructs as the actual quality of life and perception of the meaning of life were respected and theoretically worked. The thesis is theoretically based on the circle model of values system, the definitions of quality of life and approach of existential psychology and logotherapy and existential analysis to death and meaning of life phenomenon. In the empirical part of the thesis, four interviews with patients in the terminal phase of illness were analyzed with interpretative phenomenological analysis. The questions used in the semi structured interview were constructed on the basis of theories of the circle model of values by Schwartz, questionnaires of quality of life and tests discovering the personal meaning of life. We discuss similar themes emerging from the analysis of the interviews, and search for possibilities of generalizing conclusions for psychological work in palliative care. As well limitations of the research are discussed. In this thesis we bring into focus the subjective view of the terminally ill patient on his past life, and the evaluation of this lifetime from the perspective of value orientation, quality of life and meaning of life. A strong mean of positive evaluation is the possibility of perception of illness as an opportunity and the search for meaning directly in the situation of illness.
