Paramedic for emergency medicine in practice

Abstract:

Úvod: Diplomová práce se zabývá oborem zdravotnický záchranář a specializačním studijním programem zdravotnický záchranář pro urgentní medicínu. Mapuje historii oboru a srovnává současný stav oboru v České republice s jeho fungováním v zahraničí. Cíl práce: Cílem práce zjištění míry současného využití zdravotnických záchranářů pro urgentní medicínu a posouzení možností dalšího rozvoje oboru. Dalšími cíli pak jsou porovnání fungování oboru v České republice a vybraných sousedních státech, zjištění pohledu na současný výkon práce a možnosti postgraduálního vzdělávání. Metodika: Výzkum byl proveden jako kvantitativní za pomoci využití nestandardizovaného anonymního dotazníku přístupného online. Dotazník byl vytvořen ve dvou jazykových mutacích. Zpracování dat proběhlo v programu Microsoft Excel. Získaná data byla nejprve zhodnocena pomocí deskriptivní statistiky. Následně bylo pět výzkumných hypotéz statisticky ověřeno pomocí Paersonova chí kvadrát testu nezávislosti. Výsledky: Byla zjištěna výrazná disproporce mezi kompetencemi zdravotnických záchranářů v České republice a ostatních zkoumaných státech. Ta byla také potvrzena ve vnímání těchto kompetencí co do jejich dostatečnosti pro výkon povolání. Současně byl zaznamenán přínos vysokoškolského vzdělání záchranářů při možnosti jejich zásahu u pacientů se závažnějšími zdravotními komplikacemi. Naopak při posouzení překračování kompetencí nebyl zaznamenán výrazný rozdíl. Zdravotničtí záchranáři tak porušují své pravomoci ve srovnatelné míře bez ohledu na to, jak jsou široké. Závěr: Kompetence zdravotnických záchranářů v České republice jsou podstatně nižší než kompetence jejich kolegů ve zkoumaných sousedních státech. Toto konstatování platí i pro absolventy postgraduálního nástavbového specializačního studia zdravotnický záchranář pro urgentní medicínu. Jejich navýšení kompetencí oproti ostatním kolegům se jeví v kontextu možností v zahraničí jako kosmetické.

Abstract:

Introduction: The diploma thesis deals with the field of paramedic and the specialisation study programme of paramedic for emergency medicine. It maps the history of the field and compares the current state of the field in the Czech Republic with its functioning abroad. Aim of the thesis: The aim of the thesis is to determine the current level of use of paramedics for emergency medicine and to assess the possibilities of further development of the field. The other objectives are to compare the functioning of the field in the Czech Republic and selected neighbouring countries, to find out the view of the current performance of the work and the possibilities of postgraduate education. Methodology: The research was conducted as a quantitative study using a non-standardized anonymous questionnaire accessible online. The questionnaire was designed in two languages. Data processing was done in Microsoft Excel. The data obtained were first evaluated using descriptive statistics. Subsequently, the five research hypotheses were statistically tested using the Paerson chi-square test of independence. Results: A significant disproportion between the competencies of paramedics in the Czech Republic and the other countries studied was found. This was also confirmed in the perception of these competencies in terms of their sufficiency for the profession. At the same time, the benefit of higher education of paramedics was noted in the possibility of their intervention in patients with more serious health complications. Conversely, there was no significant difference in the assessment of competency overachievement. As a result, paramedics violate their competencies at a comparable rate regardless of how broad they are. Conclusion: the competencies of paramedics in the Czech Republic are significantly lower than those of their colleagues in the neighbouring countries studied. This statement is also true for graduates of the postgraduate specialisation course in emergency medicine. Their increase in competences compared to their other colleagues appears as minimal in the context of opportunities abroad.
